AFFIDAVIT OF EXPLANATION

I, MORITO JOHN BULANDRES, of legal age, Filipino, and resident of Guihulngan City, Negros
Oriental, Philippines, after having been duly sworn to in accordance with the law do hereby depose and
say;

That I failed to join the 6:00 am physical accounting at Cebu City Complex, RR Landon Street,
Cebu City, and received a memorandum dated November 12, 2020 issued and assigned by PLT ELWOOD
T EBONA;

The reason that I failed to join the 6:00 am physical accounting was because I wasn’t feeling
well the night before the said date of physical accounting and I had a full body massage to lessen the
physical heaviness I felt that night;

That I am executing this affidavit to attest to the truth of the forgoing statement for the purpose
of Passport Application and for whatever legal purposes this may serve.
