2 Unloading of Crawler crane  Failure of lifting tools &  Ensure that proper PTW are obtained from TCM

 Noncompliance with Orpic  More than 20 Ton we shall submit the lifting plan to TCM for approval.
/Tecnimont HSSE Procedure  Lifting Equipment, Tools & Shackles shall have valid third party certificate and
 Incompetent and untrained color coded, crane plot plan available
workers  Operator to complete the daily crane checklist. Visual inspection shall be
conducted for all the lifting tools and tackles by lifting supervisor prior to lifting
operation.
 Trained & certified Crane operator, Riggers shall be deputed.
 Use softeners for protecting webbing slings on sharp edges of the load

 Suspended load  Nobody is allowed to stay or pass below the suspended load
 Area shall be barricade & sign board will be displayed

 Ensure all loads secured properly


 Proper supports to be provided to avoid falling of materials
 Flag man shall be assigned for controlling unauthorized entrant.
 Swing Load  Use at least two tag lines to control the load
 Loose structural members shall not be lifted as bundled
 Swinging radius has to be barricaded
 Crane should hold the 3rd party documents
 Visual inspection to be conducted by crane operator and lifting supervisor prior to
lifting operations
 Adverse Weather conditions  Lifting activity shall be stopped whilst rain/dust/storm/poor visibility occur
 Lifting is allowed only when the wind force is 6 Beaufort (equivalent to 10.8-13.8
m/sec) or below
 Uneven ground  Level the unloading area place wood supports ready to unload the materials,
never enter below the load
 No jumping from trailer to ground.
 Riggers on trailer top must come down once the sling have stretched and ready
for lift
03 Working in Hot Environment.  Exposure to Heat  Adequate and clean cool water supplies are readily available in close proximity to
the work area.
 Frequent breaks in shaded area to allow workers to adequately recover from
work periods in direct sunlight shall be provided to all workers.
 Constant monitoring and behavioral observation of the workers is required by
Field Safety supervisors, Foreman and Supervisors to observe specifically for
signs and symptoms of heat stress.
 A shaded area large enough that all workers can take periodic breaks in an area
that is shielded from direct sunlight.
 The utilization of “buddy systems” for worker to observe each other for signs and

 Hydration Self Evaluation, Pre-employment medical examinations, Regular
rehydration, Eat healthy, Salt and Fluid Supplements.
 Workers shall be rotated within job skill activities when possible to distribute high
work load activities during extreme heat periods.
 All employees shall receive annual Heat Stress Awareness Training prior to or at
the beginning of hot season
 No work will be permitted during 12:30 to 03:30 Pm during summer time (Jun to
Aug as per MD 286/2008)
 Ensure to follow Tecnimont heat stress procedure and Flag system
Heat Index should be communicated as follows
 Above 39 degree Celsius Heat-Index, a Yellow flag will be shown in high
location(All works can be carried out)
 Above 45 degree Celsius Heat-Index , a Red flag will be shown on high location
(No heavy works like manual excavation, manual handling, shuttering etc should
be carried out)
 Above 54 degree Celsius Heat-Index , a Black flag will be shown on high location
( All works should be stopped except ongoing concreting and lifting activities)
04 Emergency at work location  Personal injury  Any incident / emergencies shall be reported to Towell first aider & site
 Traffic accident supervisor. Towell HSSE advisor will take control of situation and report to
 Incident possibility Tecnimont HSSE within 30 minutes or less.
 Fire possibility  Initial written notification of accident, incident or NM to Tecnimont HSSE within 12
 Hazards of gas leakage hrs. or less
Spillage  All personnel will be instructed on procedure to take and system of contact
(Towell/Tecnimont) should an incident occurs.
 Reporting of incidents shall be informed in daily STARRT Meeting.
 Supervisor/ Engineer in charge are responsible for leading HSE and compliance

 Fire  Maintain good house keeping
 Ensure emergency access/ egress are obstruction free
 Keep sufficient fire extinguishers
 Ensure proper cable management

 Remove all material from site immediately after completion of job
 Ensure proper shaded drinking water station with chilled drinking water for all
workers

All Hazards and the Mitigation measures reported in the JSA shall be discussed with the working crew by Supervisor/ Foreman and attendance sheet to be signed only at beginning of the work
activity. Later, on daily basis, the STARRT Meeting shall be done prior to the activity start and card to be signed by all participants.
