that would indicate why you should be cast.

Are there any “individual” one-time days where you know you will be unable to attend a rehearsal (i.e. a
wedding, an interview, etc.), during the run of rehearsals or performance dates. List and explain here. NOTE:
Haircuts and like appointments are never a reason for missing rehearsal.
Any questions about missing rehearsals, which are typically 6:30 – 9:30 Sunday through Friday (until the last
week, when there is a Saturday show). If you know you will be missing a rehearsal, talk to the director before
auditioning.

Is there anything the director should know (allergies, injuries, not willing to say certain words, not willing to say
scripted curse words, no controversial topic (be specific), no mature content, etc.), concerns? Write that below
or use other paper if necessary. It is very important that you answer this question honestly.

READ AND ONLY IF YOU COMPLETELY AGREE, SIGN: The intention is that rehearsals will be on
stage, socially distanced and with masks (occasional special clear ones which we will provide). Our
intention is that the last week of rehearsals, including at performances, we will remain socially distanced,
but perform without masks. The audience will be a considerable distance away and will be capped at 15.
This is subject to change at any time. If you are uncomfortable about any of this, it is best you not
participate.

If you understand the above, agree to participate as described (if cast), including attending all rehearsals
unless excused in advance, sign below. This also includes your guarantee that you will follow all
mitigations, which will meet or exceed recommendations of the University. If you sign below, you also
agree that you accept that if conditions deteriorate (we hope they do not) you will continue to participate
even if the show no longer has an audience (all shows will be live streamed), or worse-case scenario (we
hope this won’t happen) moved entirely on line. Safety is absolutely necessary. Failure to follow rules,
including but not limited to following all safety mitigations, missing rehearsals, failure to cooperate,
may result in immediate removal from participation in the show without warning.

I understand and agree to the above.
