    I hereby request that Disney Financial Services, LLC (or its designee) deduct from my weekly paycheck my Program
    Housing cost as described above. I understand and agree that my Program Housing cost may change from time to time
    during my program or that I may be moved to another complex or room type. I request that Disney Financial Services,
    LLC (or its designee) initiate any adjustments necessary to meet my financial obligation, and I waive all rights against the
    receiving institution for these adjustments.

    If I elect to reside in Program Housing, I understand that I am financially obligated to remain in company
    sponsored housing through the duration of my program start and end date listed on this form.
