AFFIDAVIT OF ADMINISTRATORSHIP

I, FRANCIS MYLES TEOCO MERCADO, of legal age, Filipino, and a resident of Tacloban
City after having been duly sworn to in accordance with law, hereby depose and say:

1. That I am the administrator of a parcel of land designated as Lot No. 9-C-3 covered by TCT
122-2015000411;

2. That the oft-mentioned portion of lot is now occupied by MARLENE A. ESPLANADA who
is applying for new electric service connection with LEYECO II;

3. That being the lawful owner/administrator, I am giving my consent to the application for
electric connection of MARLENE A. ESPLANADA on the residential house constructed and
standing within the portion in the aforementioned lot being administered by the undersigned;

4. That this affidavit is not contrary to law, public policy, morals and have no intention of fraud;

5. That finally I am executing this affidavit freely and voluntarily to attest to the truth and
veracity of the foregoing facts and for whatever legal purpose this may serve.
