Student Response and Assessment Tools

Lesson Idea Name: States of Changes

Grade Level(s)/Content Area: 5th grade
Content Standard Addressed: S5P1. Obtain, evaluate, and communicate information to explain the
differences between a physical change and a chemical change.

Technology Standard Addressed: Educators facilitate learning with technology to support student achievement of
the ISTE Standards for Students.

Overarching Question: Determine what factors can cause a state of change in a physical change.
Selected Technology Tool:
☐ Socrative ☐ iRespond ☐ Quizlet ☐ Plickers ☒ Kahoot! ☐ Office365 Forms
☐ Other:
URL(s) to support the lesson (if applicable):

Technology that student will use to respond to questions/prompts:

☐ Computer ☐ Hand-held student response system (like iRespond) ☒ Phone ☒ Tablet (such as iPad)
☒ Other wireless device (such as iPod Touch)
Type of session:
☐ Teacher-Paced ☒ Student-Paced
Bloom’s Digital Taxonomy Level(s):
☐ Remembering ☐ Understanding ☒ Applying ☐ Analyzing ☐ Evaluating ☐ Creating

Levels of Technology Integration:

☐ Infusion Level: Students may work at a higher Bloom’s Level, but they do not have any “Voice or Choice”
during the activity and most of the decisions are made by the teacher.

☒ Integration Level: We would like to see ALL lessons/activities reach this level. The project is student-
driven. Students have “Voice and Choice” in the activities, selecting the topic of study and determining the
technology tool to demonstrate mastery of the standard. The teacher becomes more of a facilitator.
☐ Expansion Level: The projects created are shared outside of the classroom, publishing student work and
promoting authorship. This could be reached by showcasing the project on the school’s morning
newscast, posting the project to the classroom blog, or publishing via an outside source.

Describe the instructional activities that will occur PRIOR to the SRT activity and how you will introduce
the SRT activity:
This activity will be given after the lessons and direct instruction. The lesson of changes of states will be given
and the Kahoot will be presented after to assess students’ knowledge of what they just learning. This will help
the teacher assess what needs to be gone over again.

Describe the purpose of the SRT activity (check all that apply):
☐ Assess prior knowledge ☐ Anticipatory set (Create interest in a topic) ☐ To illuminate common
misconceptions ☐ Formative assessment of content knowledge (for purpose of differentiation and
mastery for ALL students) ☒ Summative assessment of content knowledge ☐ Test preparation
☐ Survey/Poll ☐ Discussion starter ☐ “Homework” collection ☐ Other (please explain):

Briefly describe what will happen DURING the SRT activity:

Student Response and Assessment Tools
The students will need a phone or tablet or some kind of device to access the Kahoot. They will answer the
questions individually and wait for everyone to answer. After everyone submits their answers and Kahoot
shows the correct answer, the teacher will go over that answer and ask students why it is the correct answer.
Both the students and teacher will continue this process throughout the Kahoot.

Type of questions/prompts used in this activity (check all that apply):

☒ Multiple choice ☐ Multiple select ☒ True/False ☐ Yes/No
☐ Short open-ended response or fill-in the blank ☐ Longer open-ended response

If you are unable to provide a working sample of your questions, please list them below (7-10). If you
cannot provide one or two critical thinking questions in your assessment, please list a question or two you
would want students to provide an open response to below. Use your Bloom’s terms to design the
question(s):
View Kahoot button above for questions.

Immediate corrective feedback: Will you pre-select correct answers to some of all of the questions and
display correct response to the class after the SRT activity?
☒ Yes
☐ No
Why or why not?
Students will be able to have live feedback as to which questions they got right or wrong so they will know
what they need to work on.
Describe what will happen AFTER the SRT activity? How will the data be used?
The data collected from this Kahoot will not be a grade, but it will be a source for the teacher to use to know
where the whole class is on the information on the Kahoot and what they need to work on. After every
question, however many people selected each answer will be displayed so it will be a great way for the
teacher to go over common misconceptions if any problem has a large majority selecting the wrong answer.
Another Kahoot could be made to reassess students after the content is gone over again.

Describe your personal learning goal for this activity.

Using a Kahoot is a new way of assessing children. This helps the shy kids who don’t speak up as much feel as
though they can participate in a fun game and test their knowledge. As the teacher, I hope to learn where the
class is on the content provided on the Kahoot. I hope students learn and gain knowledge on any questions
they may have missed in the Kahoot.

Reflective Practice:
This Kahoot not only tests students on their knowledge, but it also gives them some examples of some higher
level thinking questions. To extend this lesson, students could create their own short Kahoots and work on
writing higher level thinking questions as well as practicing the content while writing the questions. Students
could also use a different technological tool to create higher level thinking questions and share them with
their classmates.
