AFFIDAVIT OF DENIAL

I, MARY GRACE VALERA DELA CRUZ, of legal age, Filipino, and a resident of
Pinagbarilan, Baliuag, Bulacan, after having been duly sworn to in accordance with law, do
hereby depose and say that:

1. I am a law abiding citizen who had never been involved in any case whether civil or
criminal and I had not been charged in any court or anywhere else in the Philippines;

2. In the records of the National Bureau of Investigation (NBI) it appears that a certain
GRACE DELA CRUZ was involved in the following cases:

a. Municipal Trial Court, Arayat, Pampanga, for Carnapping with Robbery in

Crim. Case No. 02-1142;
b. Regional Trial Court of Bulacan for ESTAFA in Crim. Case No. 1237-m-
2002;
c. Municipal Trial Court of Plaridel, Bulacan for ESTAFA in Crim Case No. 99-
504;
d. Regional Trial Court of San Fernando, Pampanga for Carpanning in Crim.
Case No. 12925 and for Robbery in Crim. Case No. 12926;
e. Regional Trial Court of Bulacan for Frust. Parricide in Crim. Case No. 1191-
M-2008;

3. Also, in the records of the National Bureau of Investigation (NBI) it appears that a
certain MARY GRACE DELA CRUZ was involved in a case filed before the
Regional Trial Court, Branch 85, Malolos City, Bulacan for Frust. Homicide in Crim.
Case No. 1191-M-2008;

4. I hereby deny that I am the same person mentioned in the said case and therefore, I
am not the same GRACE DELA CRUZ and MARY GRACE DELA CRUZ who
was mentioned in the said cases by NBI;

5. I have executed this affidavit to attest to the truth herein stated, and for the purpose of
informing all concerned, particularly the National Bureau of Investigation with whom
the said information gathered by herein affiant, that I am not the same person
mentioned above stated.
