     DESCRIPTION:  KEY FINDINGS:


    Client: NE1 • Poor planning of work.
    Principal Contractor: SEGC
    • In-adequate specific training.
    During the scaffolding dismantling activity, workers
    observed not complying with implementation of fall • In-competent supervision.
    protection required 100 % tie-off.

     LESSON LEARNED AND KEY MESSAGES:


    • Identification of all possible hazards and risk evaluation
    for all activities and ensure safety measures in place.
    • Clear communication of risk evaluation and hazards.
    • Enforce HSE rules and procedures strictly to avoid grave
    accidents resulting due to ignorance.
    • Pre-work training and effective toolbox talk prior to start
    an activity should be delivered and make sure awareness
    and safety measures are clearly understood by workers.
    • Empowered and adequate HSE, management
    supervision, especially for high risk activities.
    • Continuous and close monitoring by HSE & management
    personnel to immediately stop any unsafe acts or
    conditions till safety measures are in place and followed.
    • Zero tolerance for unsafe acts and unsafe conditions
    whilst carrying out any activity.
