A Pleasant Day. We, Ma Aileen O. Santucci and Lyra Baquiran, Bachelor in Nursing III students of
St. Ferdinand College are doing an undergraduate thesis entitled: THE IMPACT OF COVID-19 PANDEMIC
ON PATIENTS WITH CHRONIC KIDNEY DISEASE UNDER TREATMENT: BASIS FOR IMPROVED WELL-
BEING.

Purpose of this Quantitative study is to determine the impact of COVID 19 pandemic to CKD
patients while doing their treatment. Specifically, it aims to:
1. To assess the psychological well-being of the respondents based on their profile during the COVID 19
pandemic.
2. To identify the psychological effect of COVID 19 pandemic during dialysis treatment.
3. To be able to make recommendations to the hospital or Government develop programs or policies to
care for patients with CKD.
4. To be able to provide baseline data on the impact of COVID 19 to vulnerable patients like those with
CKD.

In connection with this, we would like to ask permission from your good office to conduct a
research study in the hospital’s dialysis center. Research questionnaires will be given to the nurse in
charge for distribution to your patients. We will be not doing face to face interview due to COVID
