TERMS AND CONDITIONS:

General Requirements:

1. The amount highlighted is calculated for net of TAX and NET of EWT, if you wish to deduct withholding
tax, you may submit a copy of certificate of VAT Exemption or form 2307.
2. Full payment of the total cost shall be paid seven days (7) upon signing of contract before
commencement of the project.
3. Payment shall be made payable to JSN CONSULTING and Advanced Training Services.
4. Payment must be deposit to our bank account, upon receipt of the deposit slip, we will send a scanned
copy of the Official Receipt thru email, and the original copy must be hands-on on the actual training
dates.
5. Upon receipt of signed/approved quotation, we will send Service Invoice for billing references
6. Approved quotation served us our binding contract upon your confirmation.

7. This quotation is valid for 30 days only and shall be revise if necessary.

Client shall responsible to provide the following: RITM

 Clients will provide Accommodations, food am/pm snacks and potable water for drinking and coffee,
logistics, such us multimedia/projector laptop/whiteboard/white board marker. Availability of Training room
 Both parties shall agree pick-up and drop off points.
o Note: If pick up are not available, we may use our company car with the reimbursement of
Php 1,500/day vice versa.
 Availability on the agreed time committed.
 Official Training time shall be 9:00am-4:00pm

JSN CONSULTING & MANAGEMENT shall provide the following:

 Soft copy of Training module will be send thru email, to be reproduce by the client.
 Certificate of attendance if Training to be signed by the Trainer & JSN Consulting Authorized signatories.
 One Trainer/one staff that will facilitate the whole day seminar
 Provide Progress Report.
 Documentation/Class Picture.

CONFEDENTIALITY AND CANCELLATION

1. The information contained on this proposal shall be treated confidential and shall not be used or disclosed
except client’s employees and JSN Consulting Management or representative.
2. Once contract and registration is confirmed any cancellation or postponement shall be made documented
and notarized and send through emails, or you may call to our office.
3. Client shall be notified JSNCON Management/Or Staff (one Week) before, in case of change of
schedule/cancellation or postponement.
4. Client will charge a cancellation fee of 30% of the total cost of the consultation or training.
