1.0 INTRODUCTION

1.1 Background

The Facilities Management (FM) at National Registration Departmen has


developed a “Services” policy to define for the government community, the
types of services provided by FM, as well as the limits to those services.

1.2 Purpose and Objective

To prepare, manage and respond to any incident or accident on the asset.


IRDRM system shall be approved by Facility Superitendant Officer (FSO)

1.3 Scope and Context

The respond to all incidents and ensure the scene of the incident is made
safe, undertake preliminary restoration of the damage, notify the
appropriate parties (including emergency services) and assist the
appropriate emergency services personnel.

1.4 Definitions, Acronyms and Abbreviations

Definitions

a. Incident Response – An organized approach to addressing and


managing the aftermath of a security breach or attack (also known as
an incident). The goal is to handle the situation in a way that limits
damage and reduces recovery time and costs.

b. Disaster Recovery – The process, policies and procedures that are


related to preparing for recovery or continuation of technology
infrastructure which are vital to an organization after a
natural or human-induced disaster.

c. Incident – Any unintended incident that may cause harm to humans,


damage to property, and the environment.

d. Accident – an incident involving actual loss through injury, damage to


assets or threats / damage to the environment.

e. Near miss – An incident that can, in other cases, result in injury,


damage to assets or harm to the environment.

f. Dangerous Occasions – Classes of hazardous occurrences as


defined in the Occupational Safety and Health Act, for example,
inverted cranes, falling diggers, lifting failures, explosions, fires, etc.

g. Serious injury – An injury resulting in a fractured bone, burning the


deck exposed to chemicals, inhaling toxic vapors. Includes

occupational illness that causes a person to be hospitalized for more
than 24 hours, unless kept for observation alone.

h. Minimal Injury – Any emergency assistance to the injury that causes


the injured person to lose normal working hours in excess of 3 days,
after days or shifts when the injury occurred.

i. Loss Time Injury – Any injury that causes an individual to lose normal
working hours beyond the day or shifter period in which the injury
occurs and must be hospitalized for more than 24 hours or be
unhealthy for more than three consecutive days.

j. Accident Frequency Rate – The ratio of the number of lost time


incidents per million working hours.

k. Incident Severity Rate – The ratio of the number of days lost to


manpower due to incidents of lost time per million working hours.

2.2 Roles and Responsibilities

a. Incident Response Personnel (IRP)


i. Direct report to Risk Manager
ii. Compiled any report related to emergencies & disaster
iii. Prepare report (weekly, monthly, quarterly, half year & yearly)
iv. Conducting training, programme & public awareness
v. Site visit, evaluation & assessment
vi. Conducting audit related to building emergencies prevention

vii. Provide tactical support for organization (emergency
preparedness)
viii. Ensure all assembly was ready for used & accessible for all
during emergency
ix. Act as floor warden for organization during emergency
x. The incident response personnel shall be responsible for:
 Notifying emergency services
 Making the scene of an incident safe
 Administering first aid if required
 Undertaking preliminary rectification works
 Providing traffic and crowd management around the
incident site
 Liaising with and supporting any emergency services
when they arrive at the scene.

b. Emergency Response Team (ERT)


i. Direct report under Incident Response Personnel (IRP)

c. JPSB’s incident response services shall include undertaking minor


repairs to damaged infrastructure until the appropriate maintenance
or emergency services arrive at the scene.

d. Any actions taken by the contractor in response to an incident shall be


in accordance with the approved Incident Response and Disaster
Recovery Plan and may include notification to the FSO and other
emergency services and/or other authorities for action and/or
dispatching a service crew.

e.The incident response personnel shall liaise with authority body that
available nearby to make safe or isolate and incident site including but
not limited to traffic management and crowd management equipment.

f. JPSB shall develop an Incident Response and Disaster Recovery


Management Plan in accordance with the contract which shall
address:

i. The performance requirements and service level requirements.


ii. How the contractor shall manage the FM Services.
iii. Requirements of emergency and incident response agencies.
iv. Processes for response planning and responding to incidents.
v. Systems and standards for all necessary actions likely to arise from
incidents requiring rapid response.
vi. Responsibilities for response measures within the contractor’s
Team.
vii. Current details of all resources (personnel, plant and materials)
available to the contractor for the purpose of responding to
incidents.
viii. Communications systems.
ix. Protocols regarding the occurrence of incidents.
x. Monitoring and recording, including response times.
xi. Emergency agencies liaison procedures.
xii. Training inclusive of regular drill.
xiii. Safety.
xiv. Risk management.
xv. Record keeping.
xvi. The requirements advise by FSO and all relevant emergency
services agencies.
xvii. Management procedures that shall be implemented in the event of
disaster occurring including processes to be followed for recovery
from disasters including temporary relocation of some operations
facilities if required.

g. The IRDRM shall be consistent and integrated with all other


management plans developed by the contractor policies and plans
developed by emergency services and the FSO.

h. The Contractor shall undertake an Incident Response Drill on an


annual basis in coordination with emergency services. The type of
activities to be included in the drill shall be determined in consultation
with emergency services and the FSO.

i. The Contractor shall review and update the IRDRM plan at least
annually to ensure continued compliance with the contract.

j. The contractor shall hold half yearly simulation exercises to simulate


the readiness of the contractor to immediately respond, mitigate, put
under control and rectify any emergency/ disaster on failure/
breakdown of critical services such as electricity supply, water supply
and pipe bursts inside the buildings.

3.0 IRDRM Methodology

3.1 Process

Emergency response team members should be thoroughly trained for potential


crises and physically capable of carrying out their duties. Team members need to
know about Evacuations hazards in the workplace and be able to judge when to
evacuate personnel or when to rely on outside help (e.g., when a fire is too large to
handle). One or more teams must be trained in:
 Use of various types of fire extinguishers.
 First aid, including CPR and self-contained breathing apparatus.

The following Table 1 –Emergency Level Matrix shall be used by the facility
Management for identifying the emergency level during emergency.

3.3 Incident Reporting

i) Reporting Accidents

Upon occurring of an accident,


a. Attend to employee promptly, injured person.

b. Determine the seriousness of the injury. Take pictures of the injury


environment if it is our employee.

c. Sent to the First Aider / Panel Doctor / Hospital

d. If the injured is not an employee, after taking pictures, get them to


describe the injury if it is possible and get them to sign on it.

e. Supervisor of the injured employee shall notify the Facility Manager


(FM) of the accident and filled up the Incident Report Form.

f. FM or appointed representative in the environmental, safety &


health committee shall conduct an investigation of the accident
internally by implementing appropriate immediate correction
Actions.

g. Submit the completed Incident Report Form to Facility


Superitendant Officer (FSO) JKR within 5 working calendar days
via electronic mail / fax.

h. FM then prepares the corrective & preventive action report within


48 hours using the Corrective & Preventive Action Report (CPAR)
to implement appropriate corrective measures upon identifying the
root cause.

i. The incident report shall be updated and upon corrective &


preventive action taken is effective and records are maintained.

j. Notification in an approved form is to be made to the respective


state Department of Occupational Safety & Health (DOSH)
whenever any accident arising out of or in connection with work

which caused any person that prevents him/her from following his
normal occupation for more than 4 calendar days.

Approved form stated as:-

1. JKKP 6 Form – Notification of Occupational Accident/ Dangerous


Occurrence.

2. JKKP 7 Form - Notification of Occupational Poisoning/Disease.

3. Whenever as a result of an accident arising out of or in connection


with work, has suffered serious bodily injuries or death, FM shall
notify the nearest Department of Occupational Safety & Health
office by the quickest means available. (E.g. telephone).

4. Respective project heads shall then communicate the results of


such incidents and corrective and preventive actions taken to other
projects within their zones.

5. Incident analysis by means of maintaining of a register of accidents


by Safety Health Officer and determining the trend of accidents for
instance in charts or other means of analysis that suits the
organization.

If medical check up is required

i. Human Resource (HR)/ FM shall handle the workers compesation,


SOCSO, medical benefits, medical leave, police report if necessary

ii. HR/ FM shall followup with the authorities, injured person and
management.

Incident Investigation

1. Incident investigation is an important tool for preventing reoccurrence


of incidents and identifying opportunities for improvements.
Incident investigations must be carried out for accidents and near
miss incidents where an injury has occurred or where there was the
potential for a serious injury or loss. The investigation should focus
on the immediate underlying and root causes of the incident.

2. The investigation must be thorough and be completed as soon as


practicable after the event with timely approach for determining the
root cause(s) of the incident.

3. The investigation will be documented using the investigation form


and if required, taking of witness or injured person statements.

4. Facility Manager if required, will review relevant documents e.g.


standard operating procedures, risk assessments, training records,
Toolbox meeting records and/or safety housekeeping checklist.

5. The investigation will ensure that if the nonconformity is identified the


appropriate and corrective and preventive actions are planned
initiated and recorded using the Corrective & Preventive Action
report form.

6. If the corrective & preventive action identifies new or changed hazards


upon reviewing the work process / activity, the FM shall fill up the
items located at the bottom of the CPAR form and submit the

CPAR form to HSE Representative review the risk.

7. Concerned personnel will then ensure the corrective actions are


implemented and review its effectiveness in a timely manner.

8. FM or concerned personnel will communicate the results during the


established environmental, safety & health committee meeting (where
available as per regulatory requirement) and to other projects under
the same division area in order to identify any trends that
might reveal areas for improvements)

3.4 Equipment and Operational Standard

JPSB are responsible to ensure all the equipment and system


that installed in National Registration Department are operational
standard. Among the equipment and facilities are:

i. Alarm system
ii. Firefighting equipment
iii. First aid facilities
iv. Sand bag
v. Spill kit
vi. Stretcher etc.

3.5 Training

JPSB wil be conduct training for their staff and client especially to be
prepare for any emergency response at least once a year.

3.6 Simulation

PSB shall undertake the simulations exercise for certain


engineering trade to measure the effectiveness of recovery to
the facilities in National Registration Department. Simulation
exercise for engineering trade such as breakdown of power
supply, cyber-attack and chiller breakdown.

i) Contractor Staff

JPSB shall establish Emergency Response Team and


conduct training which related for building safety, Fire Safety,
First Aider and etc.

ii) User/ client

JPSB plays main role to ensure all facilities in National


Registration Department are fully operational in good condition.
Therefore it is important for them to do simulation exercise.
Belows are an example of activities can be done:

 Power Supply Disruption this type of test is should perform after


hours for the least impact on regularly scheduled work. The power
shall go out for a short period of time.

 Cyber-attack the simulation model provides a user with the ability to


conduct a representative computer network and setup and execute
a series of cyber attack on certain target machines within the
network.

 Chillier Plant Breakdown The simulation exercise shall provide the


time taken to rectify and the recovery program if such breakdown
happens.

3.7 Planning and Measures

The purpose of IRDRM Planning is to ensure that the Facilities Management


team support for the National Registration Department establishes and
maintains documented IRDRM objectives and targets to implement the
Contract Obligations of Facility Management for National Registration
Department . Objectives and targets also provide a means for JPSB to
measure the effectiveness of its IRDRM efforts and improve the performance of
the IRDRM Management Plan.

3.8 Post Mortem

If any incident or event including Top Management Order for any ERP
Activation. Post Mortem will be conducted ascontinues preventive action after
incident.

4.0 Target and Output

JPSB hope there are Zero Incident during this project and will give full
commitment to ensure all workers and their contractors aware and
understand the SOP or any Safe Work Instruction.
