1. OBJECTIVE
 The purpose of this document is to provide guidance for any diving operations at
New Bong Escape Power Plant site.
2. SCOPE
 This procedure is applicable to all diving operations at New Bong Escape Hydro
Power Plant at both location as under:
o Water Intake area
o Tail race area within 50m range from plant

 CP : Competent Person.

5. RESPONSIBILITY
5.1 Senior Engineer (Mechanical)
 To ensure that the diving contractor is appropriately qualified and divers are fit to dive
as per requirement in Appendix A at the time of the activity.
 To ensure contractor to submit necessary method statement/ risk assessment prior to
commencement of the work duly endorsed/approved by chairman/co-chairman of
SOC. Approved method statement shall be made available at site of the activity.
 All required equipment to be used by the divers as per Appendix B shall be inspected
by relevant Competent Person and properly tagged.
 All pressure valves and gauges shall be checked and fitness certificate of the Oxygen
cylinder shall be demanded from the contractor.
 Discuss with diving contractor supervisor the activity required, risk assessment and
method statement prior to commencing the diving operation at the start of each

working day. The discussion shall be properly recorded.

 To provide copy of toolbox talk record sheet to diving Supervisor for recording the
session given to the diving team on the risk assessment and method statement.

5.2 Diving Contractor


 To provide sufficient ( minimum 4 divers – Diving Supervisor, Lead Diver, Diver &
Standby Diver) and suitably qualified and fit divers as per requirement in Appendix A
with sufficient equipment as per Appendix B to carry out the diving activity safely
without risk to life and health of the working party. The Company shall officially
nominate the Diving Supervisor as their Competent Person.
 To ensure that appropriate Risk Assessment and Method Statement relevant to the
diving activities performed at site are up to date and are available at the time of the
diving activity.

5.3 Diving Supervisor


 To ensure that approved method statement and risk assessment are readily available
for application of PTW for the diving activities.
 Responsible to apply & received the PTW prior commencement of the works
(Competent Person (CP)). PTW shall be made available at work site all the time.
 To ensure that each person taking part is aware of the scope of works and contents
of the diving risk based on the approved method statement & risk assessment.
 He shall NOT dive while holding the PTW.
 Responsible to conduct Toolbox Talk to all the team members on each working day.
The session shall be recorded properly as per requirement.
 To ensure Emergency Response Plan for the activities is available.
 To ensure that the divers & all equipment are fit and healthy before the
commencement of the activities.
 To ensure communication method between the Diving Supervisor and the divers are
properly established either using tug rope system or radio communication.
 To record all observations, findings, incident happen throughout the diving activities.
 To conduct pre and post diving activities.

5.4 Diving Team


 To ensure that they are fit and healthy before commencement of diving activities.
 To ensure that they understand and aware the scope of works, method of statement,
risk assessment for the activities etc before start the diving activities.

 To ensure that communication with the Diving Supervisor is established prior to
commencement of diving activities.
 To report immediately to the Diving Supervisor any problem during diving operations
or any defect on equipment used for the diving operation.
 Responsible to brief the Diving Supervisor on all observations and findings after
execution all assignment.

6. POTENTIAL HAZARD / DANGER


 Drowning
 Boat sinking or getting into distress
 Excessive water flow
 Asphyxiation
 Sudden change in weather condition
 Hypothermia
 Failure of diving equipment
 Failure of communication equipment

7. PROCEDURE
7.1 General Procedure
 The diving activity shall only be started after confirmation of the application of Permit
to Work (PTW) from Authorized Person (AP).
 All Units shall be shut down and put under “REVISION” Mode to prevent any
misoperation prior to the diving activities.
 Necessary barricade, notice and tag shall be placed at appropriate location notifying
diving operation in action (under PTW).
 The Diving Team shall report to Senior Engineer (Mechanical) at the time of arrival at
site.
 The Diving Supervisor shall discuss the diving plan, risk assessment, method
statement and Emergency Rescue Plan with his working party and communicate the
start of the diving operation with Senior Engineer (Mechanical) and SSCE on duty .
 Diving Supervisor will arrange for the first assessment dive to identify any
hazards/risks pertaining to water current or any other obstacles which identified in the
approved method statement / risk assessment.
 Any other potential hazard/risk shall be communicated to Diving Supervisor on
immediate basis.

 If the risk/hazard identified in assessment dive is HIGH, the job shall be deferred till
next suitable time keeping in view on the water current, weather and under water
condition.
 If after the first assessment, Diving Supervisor declares the condition is suitable, then
they can to proceed with the works.
 PTW for Diving Operations shall be issue and cancel on daily basis.

7.2 Hard Work – Increased Decompression


 When a diver exerts himself under pressure, his body absorbs more gas than usual
and he will require a longer period of decompression to eliminate this gas
 For diving involving hard work and / or in cold water no correction is necessary
provide that the bottom time does not exceed half the no decompression bottom time
for that depth given in Table 11 BR 2806. If the bottom time does exceed half of the
no decompression time, decompression should be that for the next longer time than
the no decompression limit
 In practice this will involve are 5 minute stop at 3 meters for all hard working dive in
cold water if the bottom time exceeds half the no decompression bottom time for that
depth
7.3 Repetitive Dives to Depths of 42M or Less
 When two dives are separated by a surface interval of 6 hour or less both dives have
to be taken into account when considering the decompression required for the
second dive.
 Dives to depths of less than 10m may follow a deeper dive without a time restriction
or requirement for further decompression.
 When dives to depths of less than 10m, or equivalent air depths of 10m, are followed
by a deeper dive all dives are taken into account when calculating decompression
stops. The only exception is when the standby diver dives to 5m to confirm he can
clear his ears. Provided the dive is completed within 5 minutes, and he is not required
to dive again until he has completed 10 minutes on the surface the dive may be
disregarded. If he is required to dive again within 10 minutes the 5m dive must be
included in the calculation of decompression stops.
 If the surface interval between dives above the limiting line exceeds six hour then no
modification of decompression is required. However, if the surface interval between

dives above the limiting line is less than 6 hours the procedure for combined dives
given in Article 4215 BR 2806 Diving Manual.

7.4 Diving Below Limiting Line


 That part of each depth section above the limiting line is the ordinary working table
where the risk of decompression sickness is negligible, diving for periods below the
line carries a greater risk of decompression sickness, and this risk increases with an
increases of duration below the line should be under taken only when a compression
chamber is available on the site and even then only when circumstances justify the
risk. The risk is in no way diminished by the use of oxygen during decompression.
 A diver who carried out a dive below the limiting line is not carry out a further dive
within 12 hours.
 A diver who has carried out a dive below the limiting line of Table 11 must remain in
the immediate vicinity of a compression chamber for a period of four hours after
completing the dive, and within four hours traveling time of a chamber for a further 12
hours. A one man compression chamber is acceptable for this requirement.
 If no compression chamber is available the diver should be kept under observation
on board for the first four hours quoted in paragraph 3 above.

8 EMERGENCY RESCUE PLAN


 The standby diver must be deployed immediately for emergency rescue operation
and the urgency is prioritized as follows:-
i. The diver is not breathing.
ii. The diver is unconscious.
iii. The diver is stressed or panicking.
iv. The diver is not responding to signal or communications.
v. The diver is trapped or fouled and cannot free himself.
vi. The diver needs support or air.
vii. The diver is lost and needs direction.
 In situations i – v, standby diver must descend immediately.
 In situations i – vii, standby diver must have clear picture of what is needed before he
descends.
 On reaching the diver distress, standby diver has to act quickly as follows:-

Immediately check if diver in distress is breathing.
o If diver in distress is not breathing:
He needs to be removed to the surface as quickly as possible. Flush the mask if
possible to increase the diver buoyancy or signal / call for him to be raised up
immediately (for shallow water). For deep water, purging the mask might allow the
diver in distress to breath again. Whilst coming to the surface make sure he is in
(head back) position and control his buoyancy. Once on the surface, pull him out the
water and start resuscitation with oxygen as quickly as possible.
o If diver in distress is breathing but unconscious:
Flush mask and recovers to surface whilst monitoring breathing ensuring his airway
is open and he breathes out. Once on the surface, administer Fist Aid to revive him
as quickly as possible or seek immediate medical assistance.
o If diver in distress is breathing & conscious:
Assess situation and help diver as required. If he is trapped, the first priority is air.
Record this and tell the surface to bring down spare air if necessary.

9 DEMOBILIZATION
 The contractor shall remove all equipment and material from work site. The
contractor must perform housekeeping to ensure that no debris or any leftover at
work site. All equipment must be properly declared at security gate prior to return.

 TNBRP personnel shall be briefed on the completion of work prior to move back.
Detail report with photos before and after work done should be submitted by
contractor to TNBRP management on completion of assignment.

Appendix 1

FITNESS REQUIREMENT FOR DIVERS & EQUIPMENT

Age Limit : Not more than 55 Years Old

Certificate : Valid Medical Certificate “Fit for Diving” from Pakistan Navy
Hospital or Medical Test undertaken at any reputable
lab/hospital (X Ray Chest, ECG, Blood Cp, Audiometry, Lung
Function, Liver Function, Renal Function, Blood Sugar, Urine)

Other Requirement :
A. Diving is not to take place within:
i. 4 hour of the consumption alcohol.
ii. 2 hours of the consumption of a heavy meal.

B. Notwithstanding A-ii, diving should not take place on an empty stomach. The
diver should, if necessary take a light snack shortly before diving.

C. Before diving is carried out, the diving supervisor must always ensure that the
efficiency of a diver has not been impaired by consumption of alcohol or drugs,
including drugs prescribed for medical treatments.
