AFFIDAVIT OF EXCLUSION AND INCLUSION

I, _______________________, Filipino, of legal age, single/married and a

resident of Lampital, Tampakan, South Cotabato, after having been sworn to in
accordance with law, hereby deposes and states that:

1. I am the present occupant and possessor of parcel of land covered by

Transfer Certificate of Title no. ___________________, located at
Lampital, Tampakan, South Cotabato, registered in the name of
________________;

2. The said registered owner already abandoned the above mentioned

property since year 2000;

3. I can no longer locate said Angelo Agustin and I have no knowledge about
his present whereabouts;

4. _______________ should be excluded as the beneficiary for the subject

property and I should be included as the new beneficiary as I am the actual
and present occupant and possessor of the said property ever since
__________ abandoned it;

5. I am executing this Affidavit to attest to the truth of the foregoing

averments and for any purpose it may serve.
