JOINT AFFIDAVIT OF DISCREPANCY

WE, __________________________ and __________________________, both of legal age,

Filipinos, with residence at Victorias City, Negros Occidental, after first being duly sworn according to
law, depose and say:

1. That we personally know the late LONIE B.BALASO who is a resident of Brgy. 1,
Victorias City, Negros Occidental;

2. That it appears on his marriage contract that his mother’s name was entered as LILIES D.
BENAL instead of LILES DEJA BENAL as per death certificate of the late Lonie B.
Balaso;

3. That the names LILIES D. BENAL and LILES DEJA BENAL, refer to one and the same
person as all the facts and circumstances appearing on the marriage contract of Lonie B.
Balaso;

4. That the mistake on the marriage contract of Lonie Balaso is merely a clerical error and
erroneous entry;

5. That we are executing this affidavit in order to state the fact that the true and correct name
of his mother is LILES DEJA BENAL.
