CONFERENCE during the last meeting where the focus is on IMPROVING those that they lack and maintaining those they are
good at
o After student’s submission, copy their self-assessment scores and rate beside it using black ink
o This (with student self-assessment and instructors assessment) will be the final copy. The assessment by the
instructor will be the basis of final score
 Print a copy of this with your signature in PDF and give a copy to the student and clinical coordinator
