Total all the score at the end divide it by 105 and multiply the result by 100. The result of this will be the one transmitted
using the transmutation table by the College of Nursing: [ ((score/105) x 100) then transmute) ]
IT IS ESSENTIAL THAT STUDENT RATE THEMSELVES FIRST, then rate them separately and do a POST CLINICAL INDIVIDUAL
CONFERENCE during the last meeting where the focus is on IMPROVING those that they lack and maintaining those they are
good at
o After student’s submission, copy their self-assessment scores and rate beside it using black ink
o This (with student self-assessment and instructors assessment) will be the final copy. The assessment by the
instructor will be the basis of final score
Print a copy of this with your signature in PDF and give a copy to the student and clinical coordinator
