MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THIS PRESENT

This agreement made and entered into this 3 rd day of June, 2021 at Vicente
D. Trinidad National High School, Iguig, Cagayan, by and between the Department
of Education, Vicente D. Trinidad National High School, Iguig District with office
address at Ajat, Iguig, Cagayan represented by JAY-AR B. ANTONIO, subject-
teacher and herein referred to as First Party;

-and-

PARENTS-TEACHERS ASSOCIATION (PTA) S.Y 2020-2021, an

organization existing in school with address at Ajat, Iguig, Cagayan represented by
Pastor Ruel C. Teh, PTA President and herein referred to as the Second Party;

-WITNESSETH-

Whereas, the subject-teacher aims to improve, teaching-learning through

the proposed study entitled “TEAM (TEACHER EXPLANATIONS IN AUDIO

MATERIAL): UTILIZING TECHNOLOGY IN INCREASING AWARENESS OF

GRADE 10 STUDENTS IN HUMAN RIGHTS”.

Whereas, the parents agreed that their children will be the respondents of
the study.

 Whereas, the student of Grade 10 are willing to be the respondents of the
study.
Whereas, the subject-teacher support in ensuring a supportive environment

for effective learning to happen as one of the DepEd mission.

Whereas, the subject teacher strongly and firmly believed that the research

will enhance students’ skills and increase their academic performance.

In connection with the above premises and considerations, the parties

hereby agree to the following terms and conditions.

1. The parents & other stakeholders will guide their children while answering
the task given to them.
2. Teacher/proponent will serve as leader to monitor the progress of the
learners.
3. The proponent must implement the action research strictly and religiously.

4. The proponent promised to maintain the anonymity of each learners and

the confidentiality of the result.

Now, therefore, for and in consideration of the foregoing promises and of the
mutual covenants herein after set forth, the parties agreed that they will help
whatever expenses incurred in improving the academic performance of Grade 10
Students of Vicente D. Trinidad National High School for School Year 2020-2021.

This memorandum of agreement shall take effect immediately upon the

approval of the action research of the proponent.
