PETITION

Petitioner, GLENBELLE MACADANGDANG GRANADO

(“Petitioner”), respectfully states:

1. This is a Petition for Application of Notarial Commission as

Notary Public under Rule III of the 2004 Rules on Notarial
Practice (Supreme Court Administrative Matter No. 02-8-13 SC).

2. Petitioner is a Filipino, married, thirty two (32) years of age, born

on 3 October 1986 in Mallig, Isabela.

3. Petitioner has been a resident of the Philippines since birth, and

presently resides at Brgy. San Jose Norte I, Mallig, Isabela. His
cellular phone number is +639153963696.

4. Petitioner desires to be commissioned as Notary Public in and

for the Province of Isabela.

5. Petitioner is private practitioner with office address at Maharlika

Highway, Brgy. Centro II, Mallig, Isabela, within the territorial
jurisdiction of this Honorable Court. The contact number of his
office are +639153963696 and +639171205152. This is where
Petitioner will be regularly conducting his notarial duties.

6. Petitioner satisfactorily completed the required course of
Bachelor of Laws (LLB.) in April 2012. Petitioner passed the
October 2012 Bar Examinations and has since been admitted to
legally practice law in the Philippines with Roll No. 62445.

7. Petitioner has paid his professional taxes for 2019 as evidenced

by Office Receipt No. 7377323 dated 8 January 2019. A copy of
the said receipt is attached hereto as Annex “A”.

8. Petitioner is a registered member of the Integrated Bar of the

Philippines (“IBP”), Isabela Chapter,1 and has duly paid his dues
with the IBP for the current year as evidenced by Petitioner’s
Official Receipt No.060794 dated 4 January 2019. A copy of the
said receipt is attached hereto as Annex “B”.

9. Petitioner attaches herein as Annex “C”, and makes an integral

part hereof, three (3) passport size colored photographs with
signature at the bottom, taken within thirty (30) days from the
date of the filing of the instant Petition.

10. Petitioner submits a picture of the office and location sketch

where he will hold his notarial services, attached hereto as
Annexes “D” to “D-2” and “E”, respectively.

11. Petitioner submits his specimen signatures, attached herewith

and made an integral part hereof as Annex “F”.

12. Petitioner also submits true impressions of his dry seal attached
hereto as Annex “G” and made an integral part hereof.

13. Petitioner is a member of the Philippine Bar in good standing

and is duly authorized to practice law under the laws of the
Republic of the Philippines, as evidenced by the following:

a. Certificate of Admission to the Philippine Bar on 29 April

2013, issued by the Office of the Bar Confidant of the
Supreme Court, a copy of which is attached hereto as
Annex “H” and made an integral part hereof;

 b. Certification that no administrative or disbarment case
against Petitioner issued by the Office of the Bar
Confidant is attached hereto as Annex “I” and made an
integral part hereof;

c. Certificate of Good Moral Character of the Petitioner

dated 15 February 2019 and Certificate of Good Standing
dated 15 February 2019, issued by two (2) executive
officers of the Integrated Bar of the Philippines – Isabela
Chapter, original copy of which are attached hereto as
Annexes “J” and “K”, respectively .

15. Petitioner respectfully manifests that he possesses all the

qualifications and none of the disqualifications for the approval of his
notarial commission.

PRAYER

WHEREFORE, it is respectfully prayed that Petitioner be

granted appointment as Notary Public for the Province of Isabela.

Other just and equitable reliefs are likewise prayed for.

and the facts alleged therein are true and correct based on my
personal knowledge or based on authentic records.

action or filed any claim involving the same issues in any court,
tribunal or quasi-judicial agency, and to the best of my knowledge,
no such action or claim is pending therein.

5. If I should hereafter learn that a similar action or

proceeding has been filed or is pending before the Supreme Court,
the Court of Appeals, or any other court, tribunal or agency, I
undertake to report that fact within five (5) days therefrom to this
Honorable Court and to the aforesaid other courts, tribunals or
