I, BERNALYN L. MANIPON, of legal age, Filipino Citizen, single and a resident of

Purok 2 Sotero Nuesa, Roxas, Isabela after having been duly sworn to in accordance with
law, do hereby deposes and says:

That I am a qualified Teacher by profession presently assigned at School of Saint

Joseph the Worker, Cabugao, Echague, Isabela and an applicant for ranking at
Nuesa Elementary School, Nuesa, Roxas, Isabela;

That I am executing this OMNIBUS FORM CERTIFICATION to assure

authencity and veracity of all documents submitted;

That I assume full responsibility and accountability of all the validity and authencity
of the documents submitted;

That I understand that the violation will automatically disqualify me from the
selection process;

That I am executing this affidavit in good faith, attesting to the truth of all the
foregoing statements as stated above, under pain of penalty, whether criminal, civil
or administrative and for all worth it may serve.
