AFFIDAVIT OF DISCREPANCY / EXPLANATION

I, CHERRY MAY BANOCNOC COLOMA, of legal age, Filipino,

single, and a resident of Refarville Subdivision, San Antonio, Roxas,
Isabela, after having been duly sworn to in accordance with law, do
hereby depose and say:

That I am registered with my name CHERRY MAY BANOCNOC

COLOMA, and that I was born on MAY 1, 1987 at GATID, STA.
CRUZ, LAGUNA, to parents EDGAR P. COLOMA and FLORENTINA
S. BANOCNOC, as per my Authenticated Certificate of Live Birth
hereto presented;

That however, since childhood, my parents had taught and

supplied me of a different spelled first name as CHERIE MAY,
which was then supplied and utilized in all of my personal
documents, especially in my scholastic records, hence the same is
now making a discrepancy to my correct and registered first
name of CHERRY MAY;

That such error is firmly believed not intentionally created to

defraud the duly constituted authorities whatsoever, but merely
for reason of carelessness and innocence on the part of my own
parents;

That I am executing this affidavit in good faith, attesting to the

truth of all the foregoing statements as above-stated, informing
the authorities concern that the name CHERIE MAY B. COLOMA,
appearing in all documents hereto presented, especially in the
herein scholastic records, is but pertaining to the same and
identical person named as CHERRY MAY BANOCNOC COLOMA, as
appearing in the hereto presented Authenticated Certificate of
Live Birth, myself, for proper identification whatsoever;

Moreover, this affidavit is being executed for the purpose of

correcting my name in my school records from CHERIE MAY B.
COLOMA to CHERRY MAY B. COLOMA, to tally with my correct
and registered data as per my Authenticated Certificate of Live
Birth issued by the National Statistics Office.

JOINT AFFIDAVIT

WE, ROXANNE MELANIE M. RIVERA and MARY ROSE VERGARA,

both of legal age, Filipinos, and residents of Bantug, Roxas, Isabela,
after having been duly sworn to in accordance with law, do hereby
depose and say:

That we are the same persons executing this affidavit;

That we personally know CHERRY MAY BANOCNOC COLOMA,

who was born on MAY 1, 1987 at GATID, STA. CRUZ, LAGUNA, to
parents EDGAR P. COLOMA and FLORENTINA S. BANOCNOC, as
per her Authenticated Certificate of Live Birth hereto presented;

That however, since childhood, the parents had taught and

supplied their daughter named above of a different spelled first
name of CHERIE MAY, which was then supplied and utilized in all
of her personal documents, especially in her scholastic records,
hence the same is now making a discrepancy to her correct and
registered first name of CHERRY MAY;

That such error is firmly believed not intentionally created to

defraud the duly constituted authorities whatsoever, but merely
for reason of carelessness and innocence on the part of the
parents;

That we are executing this affidavit in good faith, attesting to the

truth of all the foregoing statements as above-stated, informing
the authorities concern that the name CHERIE MAY B. COLOMA,
appearing in all documents hereto presented, especially in her
scholastic records, is but pertaining to the same and identical
person named as CHERRY MAY BANOCNOC COLOMA, as
appearing in the hereto presented Authenticated Certificate of
Live Birth;

Moreover, this affidavit is being executed for the purpose of

correcting the name of the subject child in her school records
from CHERIE MAY B. COLOMA to CHERRY MAY B. COLOMA, to
tally with her correct and registered data as per her
Authenticated Certificate of Live Birth issued by the National
Statistics Office.
