We spouse Carlos G. Melocoton and Nida M. Melocoton, of legal age, residing in

Poblacion 3, Midsayap, North Cotabato would like to lay down our formal complaint against
DSG Sergie H. Vistal a Philippine Army member assigned in Camp Siongco, Awang, Dos,
Maguindanao.

DSG Sergie H. Vistal used his Land Bank ATM card as a collateral for payment for the
monthly amortization of his debts. We are very disappointed when we knew that he is using
online internet banking (Landbank iAccess) to transfer his funds and we can no longer
withdraw any amount from his ATM card.

He owed ₱439,850 including the incurred interest charges for 18 months starting from
November 2019 – April 2021.

In line with this we are asking a favor from your good office to set the date for both parties
to meet and agree on the date of settlement of the debt. You can contact me anytime with this
