Affidavit of Election of Citizenship With Oath of Allegiance
2. By virtue of Article IV, Section 1(4) of the 1935 Constitution, stating that a legitimate
child of a Filipino mother and an alien father, follows the citizenship of the father, unless
upon reaching the age of majority, the child elected Philippine Citizenship, I acquired a
US Citizenship upon birth;
4. I am freely and voluntarily electing FILIPINO CITIZENSHIP by virtue of Article IV, 1987
Constitution in accordance with Commonwealth Act No. 625;
5. I am executing this Affidavit to attest to the truth of the foregoing facts and to use the same
for whatever legal purpose it may serve.
IN WITNESS WHEREOF, I hereunto affix my signature this 2nd day of June 1993 at Sampaloc,
Manila, Philippines.

OATH OF ALLEGIANCE
I, JUAN P. JHONSON, solemnly swear that I will support and defend the Constitution of
the Republic of the Philippines and obey the laws and legal orders promulgated by the duly
constituted authorities of the Philippines; and I hereby declare that I recognize and accept the
supreme authority of the Philippines and will maintain true faith and allegiance thereto; and that I
imposed this obligation upon myself voluntarily without mental reservation or purpose of evasion.
