I, JERIC RAYMOND CASIMIRO TORRES, of legal age, single, Filipino and resident of
43-K Mariana Heights 8th St., New Manila, Quezon City after having been duly sworn to in
accordance with law, depose and say:

_______________ and _____________________ a copy of my Certificate of Live Birth
is hereto attached as Annex “A”;

2. That since entering school and in my other records, dealings and activities I have used the
name JERIC RAYMOND however, when I applied for _______________ and until the
present I only used my second name Raymond being more popular on that name and for
expediency in writing my name;

3. That I am attesting to the fact that JERIC RAYMOND CASIMIRO TORRES on te birth
certificate and RAYMOND CASIMIRO TORRES on other records refer to but one and
the same person, myself;

4. That I am executing this affidavit for all intents this may be of legal worth;
