INFORMED CONSENT

Please read this document carefully. Your signature is required for your participation. You must
be at least 18 years old, be at least a college graduate, and have at least six (6) months of teaching
experience at the college level to participate. If you desire a copy of this consent form, you may
request it.

Dear Participant,

I am Cedric Joseph R. Mandap, a researcher student of San Pedro College of Davao under
the graduate school program, Master of Science in Psychology. I would like to invite you to
participate in this research survey about the psychological well-being and level of
Conscientiousness of college teachers in the teaching practice.

Before you plan to take part in this survey, it is imperative that you understand the purpose
of the study and your rights as a respondent. Please read the succeeding information carefully and
feel free to ask the researcher if you have questions or clarifications concerning your participation.

A. Description of the Study

The study is entitled, "Psychological Well-being and Conscientiousness Among College


Teachers in Davao Region". Its sole purpose is to investigate the relationship of the six factors of
Psychological Well-being and two facets of Conscientiousness of the college teachers in the Davao
Region. Psychological Well-being is a determinant of an individual's emotional health and overall
functioning. PWB is the topmost priority of several people in this pandemic, including
professionals like teachers and front liners. The researcher believes that the improvement and
stability of Psychological Well-being will allow college teachers (instructors, professors and
lecturers) to remain diligent and hardworking in their teaching practices, thus maintaining or
improving work facilitation and fostering higher student outcomes in the tertiary education.

B. Nature of Participation

You will participate in the quantitative research by providing the necessary demographic
profile and answering the two (2) survey questionnaires provided specifically the 18-item
Psychological Well-being scale by Ryff (1989) and the 20-item Big Five Aspects Scale (BFAS)
Conscientiousness scale by (IDe Young, Quilty & Peterson, 2007).

C. Purpose of the Study


The purpose of the study is to determine which among the six factors of PWB significantly
affect and predict level of Conscientiousness among college teachers in their teaching practice.
The researcher considers that the findings of the survey will provide substantial information in
order to maximize well-being in improving the diligence of college teachers (instructors,
professors, lecturers) in their field of practice. Furthermore, the development of the
conscientiousness trait will potentially result in positive outcomes reflected by improved work
facilitation and higher student outcomes in the tertiary education.

D. Confidentiality

The information to be gathered in the study will be treated with the utmost confidentiality.
Your participation on this study will be anonymous. It is not required for you to indicate your name
thus only optional. Also, coding will be used to ensure the privacy of the collected data.

E. Participants' Right

The participants have the right to refuse to participate in the study or even withdraw their
participation if they encounter a problem, distress, or uncomfortable situation at any time. They
can freely ask questions and guidance to the researcher if the statements or instructions are unclear,
ambiguous or may find difficult to understand. Also, the participants have the right to access their
results or may also request for the researcher to present the findings for them if they wished for it

The participants will also receive remuneration for their professional extension services
and assistance to help the researcher get reliable referrals to gather more sufficient data. The data
collected will be stored on the flash drive for safekeeping up until the end of time. Only the
researcher and the statistician have access to these collected data.
