1. Reference: Memo from FTO Supervisor dated August 10, 2020 with subject
Explanation (Failure to attend Monday Morning Formation)

2. ICOW the above reference the undersigned was directed to explain in writing
for failure to attend Moring Formation of the ROD FTP’s composed of 22 Police
Trainees undergoing Field Training Program at ROD’s Office on Monday 7:00AM
August 10, 2020 at ROD’s Office Camp Bagong Diwa BIcutan Taguig City .

3. Please be informed on the said date of accounting the undersigned was

advice by PCPT ROMMEL BULAN and Chief, R3 to undergo home quarantine while
awaiting for the SWAB test result

4. That the undersigned has no intention to disobey any legal orders from the
higher Headquarters .

5. Praying and Hoping that this explanation be given merit and excused for any
administrative sanction.
