Background Verification Declaration

I agree that my resume is a trusted document which confirms the authenticity of my assignments,
academic qualification, experience & other credentials and will be a determining factor in computing my
remuneration. I certify that the information given in the resume including any attachments is true and
complete

1. I understand and confirm that, the details in my resume will be used to conduct a reference
check which is a process comprising of criminal antecedents, information of previous
employments & address verification.

2. I understand that the representations made by me in the Pre - Employment Form along with
the supporting documental proofs are subject to verification as conducted in accordance with
the Tech Mahindra Business Services Ltd (‘Company’) screening mandate.

3. I hereby duly authorize the Company and/or any of its subsidiaries, affiliates, vendors, and/or
any person/organization/ third parties acting on its behalf, to verify all information furnished with
form and to generate a report accordingly.

4. I authorize the Company and their representatives to obtain all relevant information about my
education history, employment records held by any employer with whom I have previously been
associated with.

5. I unconditionally undertake to submit all the additional document/s within 7 days from the date
of demand made by the Company or its representative, failing which will entitle the Company
to suspend my employment till the period I submit the additional documents. However the
period mentioned herein will not exceed more than 30 days from the date of the demand and
Company reserves the right to terminate the employment if the additional documents are not
submitted within 30 days.

6. I understand that the information will include, the dates of employment, educational track
record, designation details, remuneration and an evaluation of my performance, competencies,
character antecedents and address details.

7. Any other pertinent information requested by the individual presenting the Company, may also
be furnished to complete the course of the verification.

8. I further understand that my appointment is conditional and subject to me clearing the

background and Reference Checks conducted by the employer or their deputed agency. If
Company has reasons to believe that I have not discontinued my previous employment contract
(if any) and Company finds that as on date my employment with the previous employer is valid
and existing while still in services with Company or any of the information provided by me is
false or incorrect, then the Company reserves unconditional rights to terminate my services
forthwith at their sole discretion without giving further notice to me.

9. I further understand that in case, if my Employment is suspended or terminated for the reasons
mentioned in para 5 and 7, then any payments made to me by Company from the Date of
Joining needs to be refunded to Company immediately and any payments due to me by
Company shall not be paid. Further the duration or period of my work in Company shall be
treated null and void ab Initio and my statutory obligations towards PF, EDLI and other
Insurances shall stand cancelled.
