Family Member Declaration Form

As the TechMBS on boarding process the prospective candidate has an obligation of declaring the details of their
family members currently employed / associated as a vendor or consultant with TechBMS.

The definition of a family member will comprises of:-

 Spouse, and parents thereof;

 Sons and daughters, and spouses thereof;
 Parents, and spouses thereof;
 Brothers and sisters, and spouses thereof;
 Grandparents and grandchildren, and spouses thereof;
 Domestic partner and parents thereof, including domestic partners of any individual in 2 through 5 of this
definition; and
 Any individual related by blood or affinity whose close association with the employee is the equivalent of a
family relationship.
