Description of incident
 Incident- trap door of 1feet x 1 feet fell on costumer phone

Investigation Summary: Today, 7th july 21, trap door got loosen and fell on the customers
mobile phone and got damage

Cause & Action: one of electrician was working yesterday night for the connection of wires of
lights and left the trap door without proper screw due heavy vibration of the moving trains here
at metro station the trap door ply of 1 feet x 1 feet of 6mm got fell on the customers mobile
phone and screen got broken, operation team has covered the expenses which got for repair of
phone display

Action-currently, we have removed the trap door and getting it done as magnetic lock and key
which will be permanently safe.
