7. ISO 45001: 2018 incorporates the Plan, Do, Check, Act stages.

Indicate which one of these stages the following arrangements (extracted from the scenario) belong
to.

(a) Active monitoring (BSAs). [ CHECK]

(b) Occupational health and safety (OH&S) objectives (for the whole site). [PLANNING]

(c) Management review (accidents with serious consequences). [ ACT]

(d) Re-visiting plans and documents (re-visiting the risk assessment). [ ACT]
(e) Competence (permit issued by a competent person). [DO]
(f) Hazard identification (hazards identified and understood by the permit issuer and contractor). [DO]
(g) Internal communication (weekly feedback meetings). [DO]
(h) Resources (providing resources for the OH&S management system). [PLANNING]

(i) Control of documented information (careful version control). [DO]

(j) Managing incidents (incident investigation to identify root causes). [CHECK]
(k) Evaluation of compliance (thorough reporting, recording of accidents). [ CHECK]
(l) Emergency response (rescued by the site’s emergency response team). [DO]

8. (a) Based on the scenario only, what are the main types of workers you should take account of in
your workplace risk assessment?
(b) One of the recommendations from your investigation was to review the risk assessment. You
decide to analyse the existing control measures for the maintenance task where the contractor was
injured.
Using the ‘general hierarchy of control’ as a framework, comment on the control measures used.

(a) 1) Young worker (young apprentice)

2) Pregnant women (expectant mother with a child due in six month)
3) Disabled people (observer with hearing difficulties.)
4) Contractor / contractor’s workers
5)Regular Employees (Site Workers)

(b) 1. Elimination- Not considered in this scenario

2.Substitution- Not considered in this scenario

3.Engineering Controls- Not much considered in this scenario, may be some controls
associated with isolation

4.Administrative Controls-

• Reasonably good administrative controls implemented

• The supervisor had briefed the contractor about isolating the hazards before starting
the work
• Specific induction training
• Provision of Permit to work system
but there was no monitoring and follow up of these controls.

5. Personal Protective Equipment -wearing of head protection while working with the
equipment. It is not clear whether head protection alone may be sufficient as a PPE. Presence
of slip hazard also indicated the need for high grip footwear.
Going by the above, it can be seen that stronger control measures like Elimination, substitution
and Engineering controls are not applied, while weaker controls like Administrative controls
and PPE are applied. These may be less effective as they are more reliant on worker
behaviour.
