AFFIDAVIT OF COHABITATION

WE, _______________________, 29 years old and__________________________,

34 years old, both single, Filipino citizens, and residents of Lower
Tingub, Mandaue City, after having been duly sworn to law, do hereby
depose and say:

1. That we have been living together as husband and wife without

the benefit of marriage since February 17, 2010;

2. That out of said union, we begot three (3) children, aged seven
(7) years old, six (6) years old, and one (1) year old;

3. That we have been living together as husband and wife for

more than five (5) years and without any legal impediment to
marry each other;

4. That we desire our union to become lawful through marriage,

thus, this affidavit is executed pursuant to the provisions of
Article 34 of the Family Code of the Philippines (Executive
Order No. 209, as amended);

5. That we are executing this affidavit to attest to the truth and

veracity of the foregoing statements.
