    Sta. Cruz, Mandaue City, Cebu after being sworn in accordance with law, hereby
    depose and say:

    1) That I am unemployed as of the present, and solely dependent to my parents;

    2) That I am currently studying at __________________________;

    3) That I execute this AFFIDAVIT to attest to the veracity of the facts stated
    herein, to comply with the requirements needed for the change of the gender
    erroneously entered in my birth certificate and, for all legal intents this may
    serve.
