WITNESSETH:
1. That I, LEONARDO GAPPI ANGELES, hereby affirm that I and my
brother Jose Gappi Angeles, are the only heirs of the deceased ROSARIO GAPPI ANGELES who died on April 4, 2018 in the City of Manila, Philippines.
2. That said deceased died intestate, without leaving any will or
Testament and without leaving any debts whatsoever.
3. That said deceased left a real estate property (lot) with
improvement, located at 3321 1st St., V. Mapa, Sampaloc, Manila, consisting of more or less SIXTY square meters and 35 decimeters (60.35) sqm., more or less, covered by TCT No. 230252, registered in the name of deceased Rosario Vda. De Angeles, and more particularly described as follows:

PSD-00-034462, being a portion of Lot 24 Block 6 (LRC) Psd-135834, L.R.C. Record No. 7680), situated in the District of Barangay of Sampaloc new Sta. Mesa, City of Manila, Island of Luzon. Bounded on the NW., along line 1-2 by Lot 13; on the NE., along line 2-3 by Lot 16, both of Block 6; on the SE., along line 3-4 by Road Lot 5, all of (LRC) Psd-135834; and on the SW., along line 4-1 by Lot 14-A of the Subd. plan. 4. That pursuant to Sec. 1, Rule 74 of the Revised Rules of Court of
the Philippines, I and my brother, JOSE GAPPI ANGELES, have agreed to divide as we do hereby divide and adjudicate EQUALLY unto ourselves, the real property, above-described.
5. That I hereby affirm that I have executed the foregoing
instrument out of my own voluntary free will without force, intimidation or violence upon my person, and that I have no claim or demand against my brother.
WAIVER OF RIGHTS
6. That I, LEONARDO GAPPI ANGELES, hereby WAIVE, RENOUNCE,
AND QUITCLAIM forever ALL my SHARE, RIGHT, TITLE, INTEREST, OWNERSHIP, AND PARTICIPATION I have over the above-described real estate property which I own in my own right, together with the improvements therein free from all liens and encumbrances thereof.
7. That I have reserved for myself sufficient property which is
necessary and adequate for my support in consonance with my standing in society.
