MOTION TO ADMIT
TC FORM 5

Oppositor, by counsel, respectfully states:

1. On 24 September 2019, Oppositor filed its Position Paper

praying for the DENIAL of the petition for issuance of safeguard
measure.

2. Oppositor has not included the TC Form 5, “Information

for the Conduct of Investigation Pursuant to the Safeguard Measures
ACT (RA 8800)” on the said Position Paper.

3. Oppositor submits the TC Form 5 to be incorporated on

its Position Paper.

WHEREFORE, Oppositor respectfully reiterates its prayer to

deny the petition for issuance of safeguard measure.

Other reliefs, just and equitable under the premises, are

likewise prayed for.
