CHECKLIST OF DOCUMENTS FOR TEACHER APPLICANTS

NOTE: Check the box whenever the document is available. Documents in the applicant’s
folder should follow the sequence as shown below.

1. CSC Form 212 (Revised 2017) in two(2) copies with latest passport size ID
pictures (with nametag and signature)
2. Certified copy of Transcript of Records
3. Special Order (if applicable)
4. Copies of Service Records, Performance Rating, and School Clearance for
those with teaching experience. If unavailable, the applicant must submit a
justification for unavailability.
5. Authenticated Report of Rating (2) copies obtained in the Licensure
Examinations for Teachers (LET)/Professional Board Examination for
Teachers (PBET)
6. Certificates of specialized training programs attended, if any (enumerate this
trainings based on the certificate)
7. Certified copy of the Voter’s ID and/or any proof of residency as deemed
acceptable by the School Screening Committee.
8. Police Clearance
9. Marriage Contract (for married woman )
10. NBI Clearance; and
11. Omnibus Certification if authenticity and veracity of all documents
submitted signed by the teacher-applicant.

Checked and verified as to the authenticity, veracity and completeness of the

data in the above-mentioned documents.
