Detailed description of my illness and other personal reasons for opting VRS.

1. I have been suffering from urticaria for the last 8 years and have been taking Anti-
histamines like Montelukast sodium, levocetirizine and atarax daily which results in
drowsiness, dizziness and blurred vision during duty hours (medical prescriptions of skin
specialists attached).

2. I have been suffering from allergic bronchitis and allergic rhinitis also. During change of
season or rise in pollution the situation aggravates so much that I have to resort to steroids
like prednisolone (medical prescriptions of chest specialists attached).

3. Because of regular use of steroids I have developed joint pain and muscle pain for which I
have been taking medicines. Because of this pain it is difficult for me to continuously sit or
stand during long duty hours (medical prescriptions of orthopaedic surgeons attached).

4. I have to look after my octogenarian father also who is confined to bed after a serious hip
injury (surgery report attached).

5. My husband is quite often on tour related to his business, so there is no one at home except
me to look after my father and children.
