Requirements Details
1. Acknowledgement of Ensure that all applicable rules and requirements are referenced in this form in order for the supplier to acknowledge and

2. Health and safety plan The applicable contract-specific/scope-of-work-specific H&S requirements that will be complied with and managed by the
tenderer. Note to Eskom H&S functionary performing the H&S tender evaluation: the H&S requirements must be scope-
of-work-specific.

3. Valid letter of good Registration with the Compensation Commissioner (COID) or a licenced mutual company or an equivalent of it (for
standing international bidders).

4. Medical Conducted in relation to the individual’s man-job specifications and the health risk assessment for each employee (including
temporary/casual and labour broker persons).

5. Performance records Occupational health and safety performance records for the past three years (fatalities and lost-time injuries).

6. Competency Do employees have the appropriate qualifications and experience for performing the assigned tasks? i.e
- First aider (If applicable)
- SHE professional (If applicable)

7. Costing for H&S Has the tenderer submitted costing for H&S? The costing must be break down of safety items and cost allocated
thereto.
