These guidelines cover the preparation of the self-evaluation performance for Sinag

Quezon right from the members to its executive board, department heads, and the whole
organization itself. Adoption of these guidelines represents a further step in the process of
harmonizing Sinag Quezon’s performance evaluation process which could be beneficial in
the organization’s improvement.

The Performance Evaluation form is composed of two (2) parts: (1) A rating form of the
member to be evaluated, and (2) the engagements, accomplishments, and assessment to the
member.

For the first part, there are ratings indicated which reflect the assessment of the actual
performance on the job. It requires sound judgment and objectivity for great importance. It is
composed of a numeral and verbal rating scale: 5 - Exceptional; 4 - Exceeds Expectation; 3 -
Meets Expectations; 2 - Below Expectations; and 1 - Needs Improvement.

Set of criteria is given which are needed to be evaluated based on the actual job
performance as for the assigned responsibilities and competence requirements of the
organization. The following are its description as well as the weighted rating it is composed.

a) ACHIEVEMENT/OUTPUT (20%). Performance is within target. Completes assigned task


without delay; able to deliver quality output even under pressure and/or time
constraints. Generates cost savings through efficient management of resources.
b) JOB COMPETENCY (15%). Displays competence in dealing with all aspects of the job;
possesses fluent, logical, and precise verbal and written communication skills; can
render sound, consistent, and transparent decisions, and logical solutions to the
problems at hand.
c) WORK ATTITUDE (15%). Works enthusiastically even in the absence of group members;
accepts new job assignment without complaint; works well with other members of the
group; can be depended upon in tight situations; demonstrates a cooperative and
supportive attitude.
d) PLAN AND ORGANIZATION (10%). Understands the big picture and aligns priorities with
broader goals; achieves project plans within timeliness and resources; anticipates and
adjusts for problems; regularly measures and evaluates performance against goals.
e) DEVELOPING PEOPLE (10%). Has the ability to train and develop people to improve their
knowledge, attitudes, and skills, to upgrade their capacity to assume higher position in
the future.
f) LEADERSHIP AND MANAGEMENT SKILLS (10%). Has the ability to motivate subordinates
and inspires team spirit even in difficult situation; commands trust and
credibility among subordinates; demonstrates creative and innovative approaches to
situations and challenges, when necessary.
g) ATTENDANCE AND PUNCTUALITY (20%). Consistently observes Sinag Quezon rules and
policies on attendance and punctuality, attends group meetings regularly, and arrives on
time.

The Weighted Rating is expected to be solved using this formula.

The second part contains the written evaluation of the performance done by the members
as to their tasks and responsibilities. It includes the:

1. Accomplishments during the evaluation period;


2. Pending Activities/Engagements during the evaluation period;
3. Activities/Engagements Not Yet Started;
4. Known Strengths;
5. Known Weaknesses; and
6. Recommendations to Improve the Performance.

These are expected to help in assessing the members as to how they were complying,
cooperating, and communicating with their respective committees.
Result System:

Note:
As per initial plan there will be recognition of the top members parang ganyan pero ang
qualified lang ay yung mga nakareceive ng exceptional to exceeds expectations na mark.

Then, for those na makakareceive ng below expectations and needs improvement,


First time- kakausapin muna ng department head
Second time- Kakausapin ng vp for membership and will be subject for further evaluation na
pwedeng lagyan pa natin ng guidelines.

For overall result of performance evaluation here is the computation:

(Positive Result)- ibig sabihin maganda ang naging performance ng kabuuhang members the
higher the better and the lower ibig sabihin madami pang things to improve
(Negative result)- ibig sabihin mas madaming members ang hindi nakapagfunction ng Mabuti
and kailangan pang iimprove at maaring hindi na muna magtuloy sa membership process.
