WITNESS STATEMENT ON OATH OF……………………

I, ..…. Male, Christian, Nigerian citizen, politician and businessman of


…………………… do hereby make Oath and state as follows:

1. That I am a registered voter in unit…… in ward……. of…….. Local


Government Area of Delta State.

2. That about 8:30am on the 9th January, 2011, I went to my polling unit at …..
where I joined the queue formed by other prospective voters awaiting the
arrival of the presiding officer and the electoral materials for the conduct of
the court fresh Governorship election.

3. That at about………., the presiding officer accompanied by security men


arrived the unit with the electoral materials to be used in the conduct of the
said election in the said polling unit.

4. That the presiding officer assisted by the two poll assistants, arranged the
voting materials after which they started the accreditation of voters.
5. That when it got to my turn the presiding officer demanded to see my voter’s
card, he used same to trace my name and particulars in the voters’ register,
where he ticked my name.

6. That indelible ink was thereafter put on my left thumb and my voter’s card
returned to me.

7. That in compliance with the announcement made by the presiding officer I


joined the other queue formed by the accredited voters.

8. That I observed that those on the queue before me went through the same
procedure.

9. That accreditation ended about 12noon after which the presiding officer
counted the number of persons accredited including myself and he entered the
figure in a form.

10. That the presiding officer then announced the voting procedure after which
voting commenced.

11. That in line with the procedure earlier announced, when it got to my turn on
the queue I was issued with a ballot paper by the INEC official and I
proceeded to the voting area and thumb printed for the candidate of my choice
with the ink pad provided by INEC for that purpose and thereafter I dropped
my thumb printed ballot paper in the transparent ballot box in the full glare of
everyone present.

12. That after I cast my vote, I moved away some distance and sat down to
observe the completion of voting, counting of votes and announcement of
result at the said polling unit.

13. That after the counting of votes upon completion of voting the presiding
officer announced the results to the hearing of everyone present and the 3 rd
Respondent won at the said polling unit.

14. That the entire electoral process in my polling unit from accreditation to the
declaration of result was done in an orderly, free and fair manner.

15. That I depose to this witness Statement on Oath in good faith, believing the
content to be true and in accordance with the Oaths Act, 2004.
