1 - PRELIMINARIES, GENERAL ITEM AND PROVISIONAL SUMS

The amounts inserted against the items in this section shall be in respect of the whole of the works.

1.01 PERFORMANCE BOND

Unit - lump sum

Measurement

(a) In the first interim payment, the contractor shall be paid either the actual premium
upon submission of receipts or the amount entered in this item whichever is lesser.
The receipts of Premium paid must be from an approved bank issuing bank
guarantee.

(b) The remaining sum (if any) shall be paid in each subsequent payment in proportion
to the progress of works.

Item coverage

(a) Performance bond in form of bank guarantee from an approved bank for duration
of coverage as stipulated in the Contract;

(b) Extension of Performance bond due to delay by the Contractor.

1.02 INSURANCES

Unit - lump sum

Measurement

(a) In the first interim payment, the Contractor shall be paid either the actual premium
upon submission of receipt or the amount entered in this item whichever is lesser.
The receipts of premiums paid must be from an approved insurance company
issuing the policies or the authorised underwriter or bank;

(b) The remaining sum (if any) shall be paid in each subsequent payment in proportion
to the progress of works.

(a) Insurance against personal injuries and damages to property;

(b) Insurance of works;

(c) SOCSO/ Workmen’s Compensation Insurance;

(d) Extension of all policies of insurance due to delay by Contractor .

1.03 SIGNBOARD

Measurement

(a) 80% on completion of the installation

(b) 20 % on removal

Item coverage

(a) excavation in any material including rock and reinforced concrete and loading into
transport, upholding the sides and keep the earthworks free of water;

(b) backfilling and compaction;

(c) support;

(d) formwork;

(e) in-situ concrete;

(f) disposal of surplus material;

(g) painting, reflectorisation and illumination;

(h) cleaning, maintaining and repairing;

(i) dismantling and removing from site;

(j) reinstatement of surfaces; and,

(k) pay all charges including obtaining permission, licences and fees, etc.

1.04 TEMPORARY BUILDING

Unit - lump sum

Measurement

(a) 60% upon establishment and completion of the facilities as specified ;

(b) 20% for maintenance which shall be made in each interim payment in proportion to
the progress of work; and,

(c) 20% on removal.

The rate shall include provision and erection, servicing and removal of the Contractor’s
office, shed, accomodation for everything required by the Contractor.

(a) Provisional of sufficient temporary building for accommodation of workers,


storage of material etc to the approval of S.O;

(b) Provision of water, sanitation, power and lighting;

(c) Application for permit from Authority including all charges, deposits and fees;

(d) Removal on completion of works of all buildings and facilities provided;

(e) Relocation of temporary building if required by the S.O.

1.05 MEDICAL SERVICES AND FIRST AID FACILITIES

Unit - lump sum

Measurement

(a) 60% upon supply;

(b) 40% to be paid in proportion to the progress of works.

Item coverage

(a) Provision of sufficient first aids kits to the approval of S.O;

(b) Maintenance and replenishment.

1.06 MOBILISATION AND DEMOBILISATION OF CONTRACTOR’S PLANTS

Unit - lump sum

Measurement

(a) 60% on completion of mobilisation;

(b) 40% on completion of demobilisation.

a) transport to site including loading and unloading at ports and Site;

(b) obtaining any import licences/permits;

(c) all taxes, import duties and other fees;

(d) assembling, erection and installation;

(e) dismantling;

(f) transport from Site including loading and unloading at ports, and;

(g) obtaining any export licences/permits.

1.07 ELECTRICAL SUPPLY

Unit - lump sum

Measurement

Payment shall be made in each interim payment in proportion to the progress of work.

Item coverage

(a) fees and charges levied by the respective authorities;

(b) comply with all safety regulations and statutes in connection therewith;

(c) provision and maintenance of electricity supply at all time, and;

(d) cutting off, dismantling and removal on completion.

1.08 WATER SUPPLY

Unit - lump sum

Measurement

Payment shall be made in each interim payment in proportion to the progress of work.

Item coverage

(a) fees and charges levied by the respective authorities;

(b) comply with all safety regulations and statutes in connection therewith;

(c) provide and maintain temporary water storage with distribution piping, and ;

(d) cutting off, dismantling and removal on completion.

1.09 SITE OFFICE

Unit - lump sum

Measurement

(a) 60 % on completion of the facilities as specified;

(b) 20% for maintenance which shall be made in each interim payment in proportion
to the progress of work and,

(c) 20 % on completion of dismantling all the facilities and reinstatement of site.

(a) preparation of site, foundation, bases and hardstandings;

(b) all necessary fences, notices and direction boards, vehicle access, parking areas and
foot paths;

(c) provision of water, sanitation, power, lighting and air-conditioning and telephone
including all fees and charges levied by the respective authorities;

(d) furniture, fittings and supply of consumable stores;

(e) depreciation;

(f) dismantling and removal of all buildings and facilities provided;

(g) disconnecting and removal of services and sealing off disused services;

(h) disposal of surplus material;

(i) reinstatement of site;

(j) moving and re-establishing portable accommodation if required.

(k) maintenance of buildings, furniture, fittings and all other facilities and services
including all necessary rental charges;

(l) replenishment of consumable stores;

(m) cleaning accommodation and pest controls;

(n) maintenance of security measures including watchman;

(o) charges for water, electricity, sewerage disposal and telephone calls;

(p) maintain all accesses for use under all weather conditions; and,

(q) relocation of site office if required by S.O.

1.10 AS-BUILT DRAWINGS

Unit - lump sum

Measurement

Payment shall be made upon submission by the contractor and approval by the S.O.

Item coverage

(a) preparation of drawings and details of completed works by licensed surveyor to the
size and format directed by the S.O. including storing all information in ACAD
(Release 14 );

(b) providing labour, tools and equipment necessary to take measurement and
obtaining such information on site as are required by the S.O. for the preparation of
as-built drawings;

(c) confirming correctness of as-built drawings; and,

(d) printing and delivery of all approved as-built drawings in hardcopies and softcopies
(CD disc) to the S.O.

1.11 TEMPORARY WORKS

Unit - lump sum

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) Temporary works required during the course of construction of permanent works;
For example, struting for trenches, cofferdams,temporary track, gantries, structures
for concrete mixing plant, block yard equipment, work yard sites,temporary sheet
piling, bunds and dewatering to keep work free of water etc;

(b) Temporary works required during the course of construction of permanent works in
order to prevent from causing damages to existing properties; For example,
temporary crossing over existing drain, temporary culvert or earth drain for
channelising existing water course etc;

(c) Temporary diversion of road and existing services such as electricity,


telecommunication, water supply, sewerage etc including locating the positions of
the utilities, protection, making good any damage caused, inspection by services
authorities etc and their reinstatement;

(d) Temporary constructional safety signs and precautionary measures;

(e) Submission of temporary work proposal with professional endorsement for


approval by the S.O;

(f) Provision and maintenance of all necessary signboards, barricades, traffic


regulating devices and guard rail;

(g) The erection at site of all necessary plant, rig, temporary works, framing,
platforms, timbers etc, and all other works necessary to support piling plant over
positions where they are required including dismantling and clearing away on
completion;

(h) Dismantling and clearing away all temporary works on completion;

(i) Indemnities to the employer against any claim by owner of neighbouring lots for
losses or damages arising from accidental flooding, siltation etc.; and,

(j) Design and endorsement by Professional Engineer.

1.12 TEMPORARY ACCESS

Unit - lump sum

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) obtaining licenses;

(b) making arrangements with owners and occupiers of land temporarily acquired and
costs arising therefrom;

(c) Preparing, amending and submitting to the S.O. and other relevant bodies,
drawings showing proposals and programme including liaison;

(d) Preparation of site;

(e) Construct temporary access and / or diversion;

(f) Ramps, structures, footways, kerbs, fencing and road marking etc. where
necessary;

(g) Modification to suit change in ground conditions;

(h) Maintenance of the access road and / or diversion;

(i) Breaking up diversions;

(j) Reinstatement of the site to its original condition;

(k) Disposal of surplus material;

(l) Providing of approved electrically operated signals for traffic control as may be
directed;

(m) Providing and maintaining landowner’s access and pedestrian right-of-way


including site preparation, footpath, fencing, signs, drainage, gates, disposal of

surplus materials and any costs arising from interference to construction
operations;and,

(n) Provision of alternative access for other contractors if their accesses are hindered
by the works in the contract.

1.13 SURVEY

Unit - lump sum

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) Survey for setting out of work, original ground levels and verifying survey data
provided by other licensed surveyor before commencement of works;

(b) Survey for verifying progress claim if necessary;

(c) Provision of survey instrument and chainmen for the use of S.O. and his
representatives;

(d) Any other survey related to the works as directed by the S.O.

1.14 PROGRESS REPORTS AND PHOTOGRAPHS

Unit - lump sum

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) Preparation and submission of progress report to PKNS MS ISO 9002 format and
at interval as directed by the S.O;

(b) Preparation and submission of progress photographs (3R size) in hard and soft
copies to format and at interval as directed by the S.O.

1.15 WORK PROGRAMME

Measurement

To be paid in proportion to the progress of work.

(a) Preparation and submission of work programme using Microsoft Project 2000 or
Primawira in hard and soft copies to format as directed by the S.O.

1.16 COMPLIANCE TO LAWS AND REGULATIONS OF VARIOUS AUTHORITIES

Unit - lump sum

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) Liaison with the Authorities in order to obtaining necessary information and
document relating to the works;

(b) Compliance with the contemporary laws and regulations of the Authorities
including all report, photographs, test etc required by the Authorities;

(c) Payment of all fees, charges and deposits to the Authorities;

(d) Arrangement for site inspection (including preparation, arrangement for the
client’s/Engineer’s endorsement on letter/ document and submission of the same )
by the Authorities and all cost incurred ;

(e) Obtaining all necessary approval and hand over to local authorities on behalf of the
Employer (with Employer’s consent); and,

(f) Keeping completed works clean for the purpose of inspection by the Authorities.

1.17 CONTRIBUTION TO CIDB

Unit - lump sum

Measurement

(a) In the first interim payment, the contractor shall be paid either the actual premium
upon submission of receipt or the amount entered in this item whichever is lesser;

(b) The remaining sum (if any) shall be paid in each subsequent payment in proportion
to the progress of works.

Item coverage

(a) Payment of contribution to CIDB and submission of receipt to the S.O.

1.18 SITE AGENT

Unit - lump sum

Measurement

Payment shall be made in each interim payment in proportion to the progress of work.

Item coverage

The rate shall include for:-

(a) the wages and other emoluments paid to the site agent inclusive of overtime
payment;

(b) the site agent working outside the contractor's normal working hours if so required
by the S.O.;

(c) costs and expenses incurred consequent upon the employment or hiring of the site
agent including payment of employee provident fund, insurances, medical and
hospitalisation;

(d) replacement of incompetent site agent; and,

(e) substitution of site agent when the appointed site agent is absent or on leave.

1.19 CLEARING AND CLEANING THE SITE UPON COMPLETION

Unit - lump sum

Measurement

To be paid full in the final payment if this item in the opinion of the S.O. is fulfilled.

Item coverage

(a) clearing and removal of all constructional debris and surplus material from
site;

(b) clearing and removal of the same above outside works limit if in the opinion of
S.O. is due to the Contractor of this works;

(c) cleaning and desilting of drain, pavement surface, road shoulder and sewers to the
satisfaction of the S.O; and,

(d) reinstate damaged area to its original condition.

1.20 MOSQUITO BREEDING

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) appointment of an approved pest control Specialist ;

(b) fortnightly inspection and control on site by the appointed pest control specialist
for prevention of mosquito related desease;

(c) Submission of inspection report to the S.O;

(d) Replacement of pest control specialist on demand by the S.O and;

(e) Payment for penalty by the Authority due to the infringement of law related to
breeding of dangerous mosquito species.

1.21 EQUIPMENT FOR S.O ( for item (a) to (e) )

Note : All equipment must be NEW

(A) FACSIMILE SERVICES

Unit - lump sum

Measurement

(a) 60 % upon delivery, including testing and installation; and,

(b) 40 % shall be made in each interim payment in proportion to the progress of work.

Item coverage

(a) delivery to S.O.'s office including testing & installation;

(b) rental and charges;

(c) maintenance, repair and regular servicing;


(d) supply and replenishment of consumable stores;

(e) depreciation and,

(f) removal.

(B) TELEPHONE / HANDPHONE

(a) 60% upon delivery

(b) 40% shall be made in each interim payment in proportion to the progress of work.

Item coverage

(a) arrangement for and cost of licenses, wave lengths and channels levied by Syarikat
Telekom Malaysia Berhad / other agencies

(b) Cost of all equipment;

(c) Installation charges;

(d) Disconnection charges and removal of all equipment;

(e) Replacement of equipment when the regular equipment is unserviceable for more
than 24 hours;

(f) Maintenance of all equipment;

(g) Rental charges and other fees (including call charges);

(h) Provision of all consumables;

(i) Regular servicing of all equipment and,

(j) Depreciation.

(C) COMPUTER SYSTEM

Unit - lump sum

Measurement

(a) 60% upon delivery

(b) 40% shall be made in each interim payment in proportion to the progress of work.

(a) delivery to S.O.'s office, including testing and installation;

(D) PRINTER

Measurement

(a) 60% upon delivery; and,

(a) delivery to S.O. 's office including testing and installation;

(b) maintenance, repair and regular servicing;

(b) supply and replenishment of consumable stores (including all lithographies);

(d) depreciation; and,

(d) removal.

(E) SAFETY BOOTS AND HELMET

Unit - Set

Measurement

It shall be the number of complete set of safety boot and helmet as accepted by the S.O.

(a) supply and delivery of safety boots and helmets to Site’s office; and,

(b) maintenance and repair.

1.21 VEHICLES FOR THE S.O.’S STAFF

1.22.1 MOTORCYCLE (NEW)

(a) 60% on delivery to site ; and ,

(b) 40 % to be paid in proportion to the progress of works.

Item coverage

The rate shall include for:-

(a) provision of a new 90 cc (minimum) motorcycle for the use of clerk-of-work on


site;

(b) delivery of vehicle to site;

(c) maintenance in a roadworthy condition and repair;

(d) petrol and all consumable parts;

(e) fuel, oil and lubricants;

(f) road tax and comprehensive insurances including renewal;

(g) depreciation;

(h) provision of a suitable replacement when the regular vehicle is unavailable for
more than 24 hours, failing which the S.O. has the right to rent a replacement
vehicle and all expenses incurred shall be borne by the contractor;

(i) keeping the vehicle clean;

(j) the motorcycle to be reverted back to the contractor on completion of works; and,

(k) collection from site when the vehicle is returned.

1.23 CLEANLINESS OF EXISTING ROAD PAVEMENT SHOULDERS AND DRAIN

Measurement

To be paid in proportion to the progress of works.

Item coverage

(a) submission of road cleanliness and dust control programme for approval by the S.O
for existing roads within the limit of works or the limit in the opinion of the S.O
ought to be included ;

(b) conducting road cleanliness and dust control programme for the whole duration of
works by an appointed personnel ;

(c) cleaning and removal of all constructional debris, accidental earth lumps, concrete
spillage , silting brought about by rain water etc, from road pavement and
shoulders ; and

(e) cleaning and removal of all silt, constructional debris, chokage etc from roadside
drain, sumps and culverts.

1.24 EXISTING SERVICES

1.24.1 EXISTING SERVICES RECORD

Measurement

To be paid in proporation to record submitted and approved by S.O.

Item coverage

(a) Search for all records to identify th nature, location and details of all underground
services.

(b) GPR scanning, ampping services and pilot trenching to accertain location and details of
underground services; and,

(c) Submission drawings.

1.24.2 SUPPORT AND PROTECTION OF EXISTING SERVICES

Unit - lump sum

Measurement

In propotion to progress of works.

Item coverage

(a) Support and proction of existing services including design and endorsement by
Professional Engineer on temporary works; and,

(b) Repair if damaged caused by conrtractor.

1.25 OCCUPATIONAL SAFETY AND HEALTH REQUIREMENTS

1.25.1 Occupational Safety And Health Officer

Unit - lump sum

Measurement

The measurement for payment for the occupational safety and health officer shall be in
proportion to the progress of works.

Item Coverage

The cost for the occupational safety and health officer shall be for the duration of the
Contract and shall include:-

(a) The wages and other enumerations paid to the site staff inclusive of overtime
payments.

(b) Cost and expenses incurred consequent upon the employment for hiring of the site
staff including payment of employee provident fund, insurance, medical and
hospitalization.

(c) Replacement of incompetent site staff.

(d) Substitution of site staff when the appointed traffic management staff is absent or on
leave.

1.25.2 Health And Safety Report

Unit - lump sum

Measurement

The measurement for payment for the health and safety report shall be the acceptance of the
health and safety report in accordance to the format and copies as stipulated in the
guideline / specification, bill of quantities and employer’s requirement.

Item Coverage

The item for health and safety report shall be in accordance with the Preambles to the Bill of
Quantities General Directions and shall include:-

(a) the costs of literatures, references and consultation in the production of the health
and safety report.

1.26 ENVIRONMENTAL PROTECTION AND ENHANCEMENTS

Payment shall be made in each interim payment in proportion to the progress of work.

Item coverage

(a) prevention of the deposition of any debris, rubbish, silt, waste materials, foul water,
chemicals etc. arising from the carrying out of the works into the existing
streams/waterways;

(b) diverting existing drains, waterways etc. encountered during the progress of the
Works without disrupting its flow system and subsequently reinstate the same if the
diversion is temporary;

(c) ensuring tyres of all vehicles leaving the site are free of mud;

(d) refrain open burning on the site;

(e) complying with requirements imposed by the relevant authority;

(f) and carrying out of the works so as to cause minimum interference to the
environment as well as existing properties;

(g) minimizing the level of pollution;

(h) providing, maintaining and removing all necessary temporary ways of access to and
within the site, as well as those from the site to public roads;

(i) preserving all earth slopes; provision of silt trap;

(j) construction adequate temporary drainage for diversion; and,

(k) abetment of noise and spread of dust.
