In pursuant to Section 8.2.b.i. & 9 of UniFAST Memorandum Circular No. 2, series of

2021., and for purposes of uniformity and standardization in the process of disbursing funds for
grantees of the Tulong Dunong Program – Tertiary Education Subsidy (TDP-TES), the CHED
Regional Office IV attached a template to be accomplished by your institution for the 1st
Semester, AY 2020-2021 to be submitted to this Office on or before July 09, 2021 with the
following documents:

1. Certificate of Registration (COR) of 1st sem AY 2020-2021 - PDF file only

2. Photocopy of ID with signature - PDF file only
3. Certified true copy of grades of 2nd sem AY 2020-2021 - PDF file only
4. Annex A - HARD COPY only
5. Annex B - HARD COPY only
