NAME DUE TO MARRIAGE

I, JETELYN N. PARACAD-AVILA, Filipino, of legal age, married and a

resident of Ugac Norte, Tuguegarao City, after having been duly sworn in
accordance with law, hereby state that:

1. My maiden name is “PARACAD”;

2. I got married to JOHN LESTER CARIDAD AVILA on June 20,

2019 at Tuguegarao City, Cagayan;

3. After marriage my name is JETELYN N. PARACAD-AVILA and all my

records have the same name;

4. I state that JETELYN N. PARACAD and the JETELYN N. PARACAD-

AVILA is the name of one and the same person and that is myself;

5. I am executing this affidavit for the purpose of updating my Professional

Regulation Commission (PRC) records;

6. I aver to the truth of the foregoing for whatever intents and purposes it
may serve.
