AFFIDAVIT OF LOSS

I, LORENA GOMEZ JUMALON, of legal age, Filipino, married, and a resident Zone 1
Upper Bantilles, Bugo, Cagayan De Oro City, after being duly sworn to an oath in accordance with
law, do hereby depose and state that:

1. I am the wife of Anastacio E. Jumalon Jr.;

2. As such I have had in my possession the Deed of Absolute Sale with Electronic Certificate
Authorizing Registration (eCAR) ,which I conveniently kept inside an envelope;

3. At present, I can no longer locate the envelope which I placed the said Deed of Absolute Sale with
Electronic Certificate Authorizing Registration (eCAR). I already looked for places that I might
place it but to no avail was not found.

4. Despite diligent efforts to locate the same, my efforts proved futile as I can no longer find
the said certificate.

5. I am executing this affidavit to attest to the fact that my Certificate of Eligibility was lost or
stolen, and can no longer be found.

6. Finally, I am executing this affidavit in order to attest to the foregoing facts and for
whatever legal purposes it may serve.

IN TRUTH WHEREOF, I have hereunto set my hand this ____________________in the City of
