ASSENT TO PARTICIPATE IN RESEARCH

KnitheWorld

1. My name is Alice Hong, and I am a graduate student at the MIT Media Lab.

2. We are asking you to take part in a research study because we are trying to learn more
about ways we can make programming and digital fabrication creative experience for
young people, through interface we’ve created.

3. If you agree to be in this study, you will be sent a link to a website and instructions on
how to use the drag-and drop block-based software. You will then join online or in-
person workshops where you and a group of others in your age will get to make knit
patterns together, and learn how to use the knitting machine. After the each of the
workshops, we’ll send you a quick survey about your experiences using the software, and
what you learned from them.

4. There is risk of data breach. However, not enough personal identifiers data (first name,
last initial, age/not birthday) are collected to minimize the risk. The data will be
encrypted and stored in a protective document on my secured computer that I use for
work.

5. Completing this study will give you the chance to learn computational thinking and
basics of knitting, and explore a new kind of interface.

6. All the workshops will be recorded and you can’t join the workshop if you don’t want to
be recorded.

7. Please talk this over with your guardian before you decide whether or not to participate.
We will also ask your guardian to give their permission for you to take part in this study.
But even if your guardian say “yes” you can still decide not to do this.

8. If you don’t want to be in this study, you don’t have to participate. Remember, being in
this study is up to you and no one will be upset if you don’t want to participate or even if
you change your mind later and want to stop.

9. You can ask any questions that you have about the study now. If you have a question
later that you didn’t think of now, you can call me (646)-872-8629, or ask me next time.
You can also call the Chairman of the Committee on the Use of Humans as Experimental
Subjects at M.I.T. at 1-617-253 6787 if you feel you have been treated unfairly.

10. Signing your name at the bottom means that you agree to be in this study. You and your
guidance will be given a copy of this form after you have signed it.
