number on your check and make the check payable to Equitable. You may submit the
payment by mail or you have the option to make a one time payment online by accessing your
policy via our website at equitable.com.
If you have any questions, please call us at 1-800-777-6510 where our Contact Center is
available to help you Monday through Thursday, from 8:30 AM to 7:00 PM EST and Friday
from 8:30 AM to 5:30 PM EST. You can also access additional support and resources, 24
hours a day, on our website at equitable.com, where there are printable forms, instructions and
other aids you will find helpful.

You may also make this payment to our website at www.equitable.com. This payment must be
made before 4:00PM Eastern Time by the due date in order to be credited on the same day.

Important: If a premium is not paid by the end of the grace period the policy will terminate except for any continuing insurance
benefits that may be provided by the law. In any event, supplemental benefits (such as those provided by accidental death or
additional term insurance provisions) do not continue after the grace period.
Equitable reserves the right to reject a payment if an unacceptable form of payment is received.
When making your payment, please remember the following:
A single personal or business check is the strongly preferred method of payment. If a cash equivalent (money order, bank draft,
cashier’s check) is used for a payment, a single cash equivalent is the strongly preferred form of payment; under certain
circumstances, Equitable may be required to report to the Internal Revenue Service receipts of cash equivalents as payment. All
payments (checks and wire transfers) must be made in U.S dollars from an account at a U.S. Financial institution. A payment in
foreign currency is not an acceptable form of payment. A traveler’s check is not an acceptable form of payment. A third party check
endorsed by the payee to Equitable is not acceptable, except rollover of a qualified plan or 1035 transfer, or Trustee check that
involves no refund. Unacceptable forms of payment will be returned to clients.
When you provide a check as payment, you authorize us either to use information from your check to make a one-time
electronic fund transfer from your account or to process the payment as a check transaction.
