Re: Relocating a Publicly-Funded Community Physiotherapy Clinic
Relocation Policy

Schedule A of the Community Physiotherapy Clinic Transfer Payment Agreement (Agreement) stipulates that
“Physiotherapy Services must be delivered only at the Recipient’s address listed in s18.1 unless otherwise agreed
to by the ministry.”

Publicly-funded physiotherapy clinics that wish to relocate and discontinue the provision of publicly-funded
services under an Agreement must notify the ministry immediately so that the Agreement can be terminated.

Publicly-funded physiotherapy clinics that wish to relocate and want to continue to be funded by the ministry to
deliver physiotherapy services under an Agreement are required to obtain prior approval from the ministry for the
proposed new location. Clinics are required to submit a written request to the ministry a minimum of 60 days
prior to the proposed relocation date. Requests submitted retroactively will not be considered since this would
constitute a breach of s 18.1 of the Agreement. The Province will terminate the Agreement of a clinic that
relocates and continues to provide publicly-funded physiotherapy services without prior approval.

The request should include documentation that clearly sets out the current location of the clinic and the proposed
location of the new clinic. In addition, it should include a transition plan for current patients and describe the
timeline and details of the transition including any gap in service delivery. The request should be sent via e-mail
to moh.physiotherapyagreements@ontario.ca.

Each clinic’s request will be reviewed independently. The ministry will consult with the LHIN in which the clinic
operates or, in cases where the proposed move crosses LHIN boundaries, with each of the affected LHINs.
Consideration will be given to:
1. the impact of the clinic move on access to services both in the original clinic location and in the new
clinic location (e.g. is there another publicly-funded clinic operating nearby) and within the LHIN(s);
2. mitigation plan proposed by the clinic for managing the transition from the new to the old location and;
3. the ability of the clinic to provide publicly-funded physiotherapy at the proposed location in accordance
with the Agreement.

Written approval must be obtained prior to any relocation. The clinic will be notified in writing within 30 days if
the proposed location has been approved or denied by the ministry in consultation with the LHIN.

In making the final decision, the Director of Health Services Branch may consider information provided by the
Applicant and such other information as the Director considers appropriate.

If approval is granted, an amendment to the existing Agreement will be prepared that will reflect the new clinic
location. The amended Agreement will be effective upon the date of relocation. The end date of the Agreement
will not change.
