We have enclosed a Declaration (which shall form part of this application form), CPF Authorisation and

the context otherwise required) for your completion.

HDB shall have the right to rely on its own records and/or the records of any other relevant authorities to
obtain or verify any information relating to or in connection with the application. In this regard, HDB also
reserves the right to write to and request information from all the applicants.

HDB collects personal data from you to administer HDB’s public housing programmes and services. We
may share necessary data with other Government agencies, unless such sharing is prohibited by
legislation. This is to enable us to serve you in the most convenient, efficient and effective way. We will
NOT share your personal data with non-Government entities, except where such entities have been
authorised to carry out specific Government services.

 DECLARATION AND CPF AUTHORISATION
I/We declare that :-

1. All the particulars given in this Application Form are true and correct, and I/we have not suppressed any
information.
2. I/We will notify the HDB immediately in writing of any changes in the information provided in this Application
Form and/or of any changes affecting my/our eligibility to buy an HDB flat.
3. I/We have read, understood and accepted the General Conditions for Purchase of a Flat under the Build To
Order Exercise. I/We fully agree and accept that the HDB shall have the right to introduce new terms and
conditions and/or revise the General Conditions at any time, even if it will adversely affect my/our eligibility to
apply for an HDB flat.
4. I/We understand and accept that if I/we am/are deemed ineligible by HDB to purchase an HDB flat under any
of the priority Schemes, the HDB shall have the right to process the application under the Public Scheme, if
applicable.
5. Neither I nor any persons listed in this application have any outstanding debts due to the HDB.
6. The HDB has the right to reject/cancel my/our application and forfeit any deposit or fees as determined by
HDB if:
a) I/We, or any person listed in this application, is deemed by the HDB to be/become ineligible to purchase an HDB flat,
b) I/We, do not have enough cash and/or CPF monies to pay the requisite deposit at the time of signing the Agreement
for Lease,
c) I/We, fail to pay any cash sum payable by me/us as determined by the HDB.
d) Any information given in this Application Form is incorrect and I/we have suppressed any information.

CAUTION: IN ADDITION TO OTHER REMEDIES, THE HOUSING & DEVELOPMENT ACT (CHAPTER 129) PROVIDES
THAT ANY PERSON WHO MAKES A FALSE STATEMENT IS LIABLE ON CONVICTION TO A FINE NOT EXCEEDING
$5,000/- OR IMPRISONMENT FOR A TERM NOT EXCEEDING 6 MONTHS OR BOTH.

If I/We have made any false statements in the application, HDB may debar me/us from buying a flat* or taking over
ownership of a flat* by way of transfer, being included as an essential occupier of a flat* or taking up an HDB housing loan
for a period of 5 years from the date of cancellation of the application or the date of approval to compulsorily acquire the
HDB flat. Any existing application registered in the HDB’s rental waiting list will also be cancelled.
*includes a unit to be bought under the Design, Build and Scheme or the Executive Condominium Housing Scheme

7. I/We have the mental capacity to understand the nature and consequences of the flat transaction we are
entering into. I/we have not been scheduled for any mental health assessment.
8. I/We undertake to abide by the HDB's policies, terms and conditions as may be laid down from time to time.

a) the HDB to have access to and obtain from the CPF Board any information relating to my/our CPF account(s) and to
release my/our particulars to CPF Board for any purpose relating to or in connection with this sales application.
b) the CPF Board to release any information regarding my/our CPF account(s) to the HDB for any purpose relating to or
in connection with this sales application.

c) the HDB to disclose any of my/our information obtained from the CPF Board pursuant to this authorisation to my/our
co-applicant(s) in this sales application.
