Demobilization Audit Checklist

Purpose

To ensure the demonization of equipment and materials is complete and that the site condition is acceptable.

Scope

Applies to the demobilization of all contractor camps and sites.

Demobilization
Has a de-mobilization inspection of equipment, loads, and
transports been completed?
Do all of the loads have the appropriate manifests and
permits, as required?
Have all materials, equipment and vehicles been removed
from the site?

Wellhead and Cellar

Has the wellhead been greased and is free of leaks?

Have isolation caps been removed from the hydraulically
actuated wellhead valves?
Have all fluids been removed from the well cellar and disposed
of?
Have wellhead barriers been replaced on the well cellar(s)?

Well Pad
Is the pad in good condition and is free of sand, holes,
damage, debris, garbage, etc.?
Has all waste been removed from the well pad and properly
disposed of (empty drums, chemical waste, used oil, etc.)?
Have all spills been remediated or contaminated soil has been
removed for disposal?
Has security wire been removed from the security berm and
disposed of?
Has the security berm been replaced at the pad entrances to
restrict access?

Pits
Have fluids been removed from all pits and disposed of?

Have all pit liners been removed and properly disposed of?
