STI faculty members have the responsibility to provide careful evaluation of students and timely
assignment of appropriate grades. It is expected that the judgment of each faculty member is
authoritative, and the final grades assigned are correct.

STI recognizes that there are cases where it is necessary for a faculty member to request a change of
grade for a student. The purpose of this policy is to establish guidelines and procedures in the facilitation
of grade change and to ensure that the request for such is well-documented and properly justified.

Internally, management may make changes to the new Change of Grade Guidelines given by the Head
Office to improve the effectiveness of its operation and the policy’s intent. In this regard, any employee
who wishes to suggest improvements about this policy may forward suggestions to the Academic Head
and School Administrator.

With this letter, this is to make amend on the revisions of grades. Below, is the list of students whose
grades will be changed not in the system (One STI Application and Oracle) but through their Form 138.
