PETITION

Petitioners, thru the undersigned counsel and to this

Honorable Court, respectfully state that:

1. Petitioners are husband and wife, Filipinos, both of legal

age and residents of ________ Panaon, Misamis
Occidental Attached herewith is the copy of the
petitioners’ Marriage Contract marked as Annex A;

2. Petitioners hereby desire to jointly adopt a child named

IRYLMAR GLEE TIBOR (IRYLMAR for brevity) who was
born on August 18, 2019 at Oroquieta City. Copy of the
child’s Certificate of Live Birth is attached as Annex B;

3. IRYLMAR is the biological son of JENNY B. TIBOR

(JENNY for brevity), and JEFFREY MALALIS (JEFFREY
for brevity). However both parties, JENNY and JEFFREY,
did not see each other anymore and can no longer be
located by JENNY since her pregnancy until she gave
birth to IRYLMAR;

4. JENNY has already given consent to this adoption.

Attached herewith is the Affidavit of Intent and Consent
to Adoption executed by JENNY marked as Annex C;

5. The child to be adopted is already a little over eleven(11)

months old now;

 6. Petitioners were taking care already the child from his
conception through providing his mother Jenny vitamins
and regular check-up to a doctor until his birth and up
to present.

7. Petitioners have been giving care, love, support and

exercising parental authority over the child for quite
some time;

8. IRYLMAR is not disqualified by law to be adopted;

9. IRYLMAR does not own any personal or real property;

10. Petitioners have the legal capacity to adopt and are in

possession of all civil and legal rights and have never
been convicted of any crime involving moral turpitude;

11. They are emotionally and psychologically capable of

caring for IRYLMAR;

12. The child is in good health condition evidenced by the

attached health certificate marked as Annexes E and E-
1;

13. Petitioners are financially capable to adopt IRYLMAR.

Copy of their financial statements is attached as Annex
F;

14. Petitioners are in a position to support and care for pre-

adoption services as required by Republic Act No. 8552,
Section 4;

15. Also attached are three (3) letters of character

references as Annexes G, H and I and the family
photographs of petitioners and the child to be adopted
marked as Annexes J and K;

16. Upon finality of the decree of adoption, IRYLMAR GLEE

TIBOR shall be known and registered in the Office of the
Local Registry of Oroquieta City as IRYLMAR GLEE
BULANDRES following the petitioners’ surname.

WHEREFORE, this Court is prayed that upon due notice

and hearing, judgment be entered:

 1. Decreeing the adoption of IRYLMAR GLEE TIBOR in
favor of petitioners;

2. Changing the name of IRYLMAR GLEE TIBOR to

IRYLMAR GLEE BULANDRES and the same be entered
in the Civil Registry;

3. IRYLMAR GLEE BULANDRES be declared the child of

herein Petitioners.

Other just reliefs are also prayed for.
