I, ROGELIO A. SANTOS, 49 years old, married, Filipino, and
presently residing at 645 Aranga St., Sampaloc, City of Manila, after having
been duly sworn to in accordance with law, hereby depose and state that:

I am a professor at Taguig City University;

One of my student is JAKE R. VERTUDEZ who is under my PE -

Individual/Dual Sports subject;

On 4 January 2019, I placed Unofficially Dropped for his grade last 1 st

semester, academic year 2018-2019 when I accidentally misplaced his records
on my files so I was opted to give him the above-mentioned grade, upon
retrieving, he passed his midterm and final practical exam, in truth and in
fact, his final grade should have been Two (2);

That I am executing this Affidavit to effect the change in her grades

FROM Unofficially Dropped TO two (2) for all legal intents and purposes this
may serve.

in fact, it should have been One point fifty (1.5);

That I am executing this Affidavit to effect the change in her grades

FROM Two point fifty (2.50) TO One point fifty (1.5) for all legal intents and
purposes this may serve.

 AFFIDAVIT
I, JOEY DELA CRUZ y SAMAR, 37years old, married, Filipino, and
presently residing at Block 147 Lot 28, Brgy. Central Bicutan, Taguig City ,
after having been duly sworn to in accordance with law, hereby depose and
state that:

I am an Angkas Driver with ID No. F10902746;

I usually place the said ID in my vest in the ID slot located at the upper
right shoulder.

Last Tuesday, 03 March 2020, around 8:00 in the evening while I was
traversing East Service Road, I noticed that my ANgkas ID was no longer in
place. I tried to look for it but to no avail.

Efforts to retrieve the said ID, ended in futility and to no avail.

Thus, I considered the said ID lost and beyond recovery;

I am executing this affidavit all legal intents and purposes this

may serve.
