ENGAGEMENT AGREEMENT

I.
IDENTIFICATION OF PARTIES

This engagement agreement is freely made and entered into this

_____ day of _________, 20___ in Quezon City, Metro Manila by and
between:

Atty. Mikhail Valodya M. Tupaz, of legal age, married, Filipino with

permanent address at 37 Sunshine Street, Rema Village, Tandang Sora,
Quezon City, hereinafter referred to as “Attorney”;

-and-

___________________________, of legal age, Filipino, with

permanent address at __________________________________________,
herein referred to as “Client”.

WITNESSETH

WHEREAS, the Attorney is a member in good standing of the

Philippine Bar and is willing to offer his legal services to the client;

WHEREAS, the Client is in need of the services of a lawyer to

recover/claim death benefits of her deceased husband;

NOW THEREFORE, for and in consideration of the foregoing and

mutual covenants herein contained, the Attorney has and by these presents
agree to render legal services in favor of the Client who hereby accepts the
same subject to the following terms and conditions:

LEGAL SERVICES TO BE PROVIDED

The legal services to be provided by the Attorney to the Client are as

follows:

i. Demand from the employer of the deceased husband of the

Client any and all monetary proceeds arising from the death of
the said husband and owing to the client;
 ii. Demand from a shipping agency / insurance provider of the
deceased husband of the client any and all monetary proceeds
arising from and owing to the client;
iii. Keep client informed of the progress of the claim and
developments surrounding the claim; and
iv. Promptly respond to the Client’s inquiries and communication as
soon as possible;

III.
RESPONSIBILITIES OF THE CLIENT

i. Divulge to the Attorney all necessary facts regarding the claim

truthfully and openly;
ii. Keep the Attorney reasonably informed of the developments and of
the Client’s address, telephone and cellphone numbers, as well as
the Client’s whereabouts as may reasonably needed;
iii. Timely make payments as required by this Agreement.

IV.
COMPENSATION

The Client will pay the Attorney for the legal services provided under
this Agreement, as follows:

i. A contingency arrangement whereby the Client shall pay the

Attorney fifteen (15%) of any and all proceeds from the recovery
of any monetary proceeds owing to the Client as the beneficiary
of her deceased husband;
ii. Any and all miscellaneous expenses, such as but not limited to,
documentation expenses and transportation expenses shall be
shouldered by the Client.
